Bacterial endocarditis is a serious infectious disease affecting the inner lining of the heart, known as the endocardium, and most significantly the heart valves in dogs. This condition occurs when bacteria colonize the heart valve surfaces, forming growths called vegetations composed of bacteria, inflammatory cells, fibrin, and platelets. These infected vegetations damage valve tissue, impair normal valve function, and serve as a source for bacteria to continuously enter the bloodstream, causing systemic illness. Bacterial endocarditis represents a medical emergency requiring aggressive treatment, as untreated cases progress rapidly and carry high mortality rates.
The development of bacterial endocarditis involves bacteria entering the bloodstream through various routes and subsequently attaching to heart valve surfaces. Common sources of bacterial entry include severe dental disease with infected gums, skin infections, urinary tract infections, bone infections, and contaminated intravenous catheters. Once bacteria reach the heart, they preferentially attach to areas of turbulent blood flow or pre-existing valve damage, where the disrupted endothelial surface provides an ideal attachment site. The aortic and mitral valves on the left side of the heart are most frequently affected in dogs, likely due to the higher pressure environment and greater stress these valves experience compared to right-sided valves.
The impact of bacterial endocarditis on a dog's health extends far beyond the heart itself. Infected vegetations continuously shed bacteria into the bloodstream, causing persistent bacteremia that spreads infection to multiple organs throughout the body. Fragments of vegetations may break off and travel to distant sites, causing embolic events that block blood vessels supplying the kidneys, spleen, brain, and other organs. Progressive valve destruction leads to regurgitation, where blood leaks backward through incompetent valves, ultimately causing congestive heart failure. The combination of systemic infection, organ damage from emboli, and cardiac dysfunction makes bacterial endocarditis a life-threatening condition requiring prompt, intensive treatment.
Treatment of bacterial endocarditis involves prolonged courses of intravenous and oral antibiotics selected based on culture and sensitivity testing whenever possible. Dogs typically require four to eight weeks of antibiotic therapy, often starting with intravenous administration in a hospital setting before transitioning to oral medications at home. Supportive care addresses cardiac dysfunction, fever, pain, and other complications. Despite aggressive treatment, bacterial endocarditis carries a guarded to poor prognosis, with mortality rates reported between 50 and 75 percent in various studies. Early detection before severe valve destruction occurs, identification of the causative organism to guide antibiotic selection, and absence of major embolic complications improve survival chances. Prevention through maintaining excellent dental health and promptly treating infections elsewhere in the body represents the most effective strategy for this devastating disease.
