Atrial fibrillation is a cardiac arrhythmia characterized by rapid, chaotic electrical activity in the heart's upper chambers, the atria, resulting in an irregular and often abnormally fast heart rate. In this condition, the normal organized electrical impulses that originate from the sinoatrial node are overwhelmed by multiple disorganized electrical wavelets circulating through the atrial tissue. This chaotic electrical activity prevents the atria from contracting effectively and causes the ventricles to beat irregularly as they respond to the random impulses reaching them from above. Atrial fibrillation is the most common sustained arrhythmia encountered in dogs, particularly affecting large and giant breed dogs with underlying heart disease.
The mechanism underlying atrial fibrillation involves complex electrical and structural changes in the atrial myocardium that create conditions favorable for arrhythmia initiation and maintenance. Normal atrial contraction requires coordinated electrical activation spreading smoothly across the tissue. In atrial fibrillation, multiple small reentrant circuits or rapidly firing foci generate disorganized electrical activity that fragments into wandering wavelets. The atria quiver rather than contract effectively, losing their ability to pump blood actively into the ventricles. While the ventricles continue to fill passively, the loss of atrial kick reduces cardiac output by approximately fifteen to twenty-five percent. The irregular ventricular response creates the characteristically irregular pulse that veterinarians detect during examination.
Atrial fibrillation in dogs typically occurs in one of two contexts with different implications for management and prognosis. Secondary atrial fibrillation develops as a consequence of underlying structural heart disease, most commonly dilated cardiomyopathy in large breeds or advanced mitral valve disease. The underlying condition causes atrial enlargement and stretching that creates the substrate for arrhythmia development. Lone atrial fibrillation, occurring without detectable underlying heart disease, is seen primarily in giant breed dogs whose large hearts may be inherently predisposed to this arrhythmia even without structural abnormalities. The distinction between these presentations influences treatment goals and expected outcomes.
Management of atrial fibrillation focuses primarily on controlling the ventricular rate rather than restoring normal sinus rhythm, which is difficult to achieve and maintain in dogs with established arrhythmia. Rate control medications slow the conduction of impulses through the atrioventricular node, reducing the rapid ventricular response that impairs cardiac function. In some cases, electrical cardioversion may be attempted to restore normal rhythm, though recurrence is common. Treatment of any underlying heart disease is essential to address the root cause and optimize cardiac function. Regular monitoring by a veterinarian or veterinary cardiologist helps ensure adequate rate control and early detection of disease progression. With appropriate management, many dogs with atrial fibrillation maintain acceptable quality of life, though the arrhythmia generally indicates significant cardiac disease requiring ongoing attention.
