Autoimmune hemolytic anemia, commonly abbreviated as AIHA, is a serious hematological condition in which a dog's immune system mistakenly identifies its own red blood cells as foreign invaders and mounts an immune response to destroy them. This inappropriate immune attack leads to rapid and sometimes catastrophic depletion of red blood cells, resulting in severe anemia that can become life-threatening within hours to days if not promptly recognized and treated.
The condition is one of the most common autoimmune diseases diagnosed in dogs and represents a significant clinical challenge due to its unpredictable onset, variable severity, and potential for fatal complications. AIHA can present as a primary (idiopathic) condition in which no underlying trigger is identified, or as a secondary condition triggered by infections, medications, neoplasia, or other diseases that stimulate aberrant immune activation against red blood cells.
Red blood cells are essential for transporting oxygen from the lungs to every tissue in the body. When these cells are destroyed faster than the bone marrow can replace them, the resulting oxygen deprivation affects every organ system. Dogs with AIHA can deteriorate rapidly, progressing from apparently normal health to collapse within a very short timeframe, making early recognition of clinical signs critically important for survival.
AIHA accounts for a substantial portion of emergency presentations at veterinary hospitals, and despite advances in treatment protocols, mortality rates remain significant, ranging from 20 to 70 percent depending on the severity at presentation and the presence of complications such as thromboembolic disease. Understanding this condition, its triggers, and the principles of management is essential for dog owners and veterinary professionals alike.
