Proptosis is a serious ophthalmic emergency in which the eyeball becomes displaced forward out of the eye socket, with the eyelids trapped behind the globe. This traumatic condition occurs when sufficient force is applied to the head or facial region, causing the eye to prolapse anteriorly beyond its normal position within the orbit. In cats, proptosis is less common than in brachycephalic dog breeds due to cats' deeper orbital structures, but when it does occur, it represents an urgent veterinary emergency requiring immediate intervention. The condition can result in permanent vision loss, loss of the eye itself, or even be life-threatening if associated with severe head trauma.
Proptosis in cats typically results from significant blunt force trauma to the head or face. Common causes include motor vehicle accidents, falls from height, animal attacks from dogs or other predators, and severe facial impacts. Unlike brachycephalic dog breeds whose shallow orbits make proptosis more likely with minimal trauma, cats generally require substantial force to displace the eye. This means that when proptosis occurs in cats, there is often concern for additional injuries including skull fractures, brain trauma, or damage to other facial structures. The severity of the underlying trauma significantly impacts prognosis.
The impact of proptosis on the cat's health extends beyond the obvious eye displacement. Once the eyeball is displaced anteriorly, the eyelids snap closed behind the globe, trapping it outside the orbit. This prevents normal blinking and tear distribution, causing rapid drying and damage to the cornea. The blood supply to the eye may be compromised, and the optic nerve and extraocular muscles are stretched and potentially torn. Without immediate treatment, irreversible damage to the eye occurs within hours. The cat typically experiences significant pain and distress, and associated injuries may pose additional life-threatening concerns.
Treatment for proptosis must be initiated as an emergency, ideally within minutes to hours of the injury occurring. The goal is to replace the eyeball into the orbit if viable and address any concurrent injuries. Success depends largely on the severity of damage at presentation, the duration of exposure, and the extent of trauma to the optic nerve and blood supply. Even with immediate optimal treatment, many cats with proptosis will lose vision in the affected eye, and some will require enucleation if the eye cannot be saved. Early veterinary intervention maximizes the chance of a positive outcome, making this a true time-critical emergency.
