Phaeohyphomycosis is a collective term for infections caused by dematiaceous or darkly pigmented fungi that exist naturally in soil, decaying plant matter, and wood throughout the environment. These fungi contain melanin in their cell walls, which gives them their characteristic brown or black coloration and contributes to their virulence. In cats, phaeohyphomycosis typically manifests as a subcutaneous infection, though it can also affect the nasal passages, central nervous system, and other internal organs in more severe cases. The condition occurs worldwide and represents one of the opportunistic fungal infections that veterinarians must consider when cats present with chronic, non-healing nodules or masses, particularly those that have failed to respond to antibiotic therapy.
The development of phaeohyphomycosis in cats typically begins when fungal organisms gain entry to the body through wounds, most commonly penetrating injuries from thorns, splinters, or other foreign objects contaminated with soil or plant material. Outdoor cats face significantly higher risk due to their increased exposure to environmental sources of these fungi and greater likelihood of sustaining wounds while exploring. Unlike some fungal infections that spread through inhalation of spores, phaeohyphomycosis most often develops through direct inoculation of organisms into the subcutaneous tissues. Once established in the body, these fungi can be extremely difficult to eliminate because their melanin content protects them from host immune defenses and some antifungal medications.
The impact of phaeohyphomycosis on affected cats varies considerably depending on the location and extent of infection. Subcutaneous forms may cause localized discomfort and disfigurement but are generally not life-threatening if properly treated. However, infections involving the nasal passages can cause chronic nasal discharge, facial deformity, and significant quality of life impairment. Disseminated infections affecting the brain or other internal organs carry a much graver prognosis and may prove fatal even with aggressive treatment. The chronic, progressive nature of this infection means that early diagnosis and intervention significantly improve outcomes, while delayed treatment allows the infection to become more extensive and resistant to therapy.
Treatment of phaeohyphomycosis requires a combined approach using surgical excision of infected tissues along with prolonged antifungal medication. Complete surgical removal of affected tissue, when anatomically possible, provides the best chance for cure. Antifungal therapy typically continues for months after surgery to eliminate any remaining organisms and prevent recurrence. The prognosis for cats with localized, surgically excisable disease is generally favorable, though treatment is lengthy and requires significant owner commitment. Cats with more extensive or internal infections face a more guarded prognosis. Veterinary expertise in diagnosing and managing fungal infections is essential, as phaeohyphomycosis can easily be mistaken for bacterial infections or neoplasia, leading to inappropriate treatment and disease progression.
