Foreign body reactions, particularly from plant material such as grass awns, produce chronic draining tracts that can mimic actinomycosis. Neoplastic conditions including soft tissue sarcomas and mast cell tumors may ulcerate and drain, resembling infected lesions. Immune-mediated conditions occasionally produce chronic skin lesions. Complete diagnostic workup is necessary to confirm actinomycosis and exclude these alternatives.

Potential complications of actinomycosis include conditions that develop from spread of infection or consequences of chronic disease. Osteomyelitis develops when infection extends into bone, causing bone destruction and potentially chronic pain. Pyothorax or empyema can develop from thoracic actinomycosis, with accumulation of infected fluid in the chest cavity. Peritonitis may occur with abdominal involvement. Secondary bacterial infections with other pathogens can complicate wounds. Chronic weight loss and debilitation occur with prolonged illness. Monitoring for these complications allows for early intervention and appropriate modification of the treatment approach.
