While relatively uncommon in cats, nocardiosis represents an important diagnosis to consider when cats present with chronic, non-healing wounds, draining skin lesions, or respiratory disease that fails to respond to standard antibiotic therapy. The infection can remain localized to skin and subcutaneous tissues or disseminate to affect multiple organ systems, with severity ranging from manageable local disease to life-threatening systemic infection.
Nocardia species are gram-positive, partially acid-fast bacteria that belong to the order Actinomycetales, sharing characteristics with both typical bacteria and mycobacteria. Cats become infected through wound contamination with soil or through inhalation of the organisms. Once established, Nocardia bacteria cause chronic, progressive infections characterized by granulomatous inflammation and abscess formation. The organisms' ability to resist killing by phagocytic cells and evade immune responses contributes to the persistent, difficult-to-treat nature of these infections.
The clinical impact of nocardiosis in cats depends largely on the route of infection and extent of disease spread. Cutaneous nocardiosis, typically resulting from wound contamination, produces chronic draining lesions, subcutaneous nodules, and mycetomas that persist despite standard wound care. Pulmonary nocardiosis follows inhalation of organisms and can cause pneumonia or pyothorax. Disseminated disease affecting multiple organs carries the most serious prognosis. Any form of nocardiosis requires prolonged treatment, and delay in diagnosis often allows disease progression that worsens outcomes.
Treatment of nocardiosis requires extended antibiotic therapy, typically lasting months, using specific antimicrobial agents effective against these organisms. Standard first-line antibiotics commonly used for typical bacterial infections are often ineffective, making accurate diagnosis essential for appropriate treatment selection. With early diagnosis and adequate treatment duration, many cats with nocardiosis can achieve complete cure, though the extended treatment period demands considerable owner commitment. Understanding this condition helps cat owners recognize suspicious signs and seek timely veterinary evaluation, improving chances for successful outcomes.
