Cats with chronic outer ear infections are at significantly elevated risk for developing middle ear involvement, and management of otitis media typically requires concurrent attention to any outer ear component. Inner ear infection, or otitis interna, represents further progression of disease from the middle ear and shares many symptoms with otitis media but typically causes more severe vestibular dysfunction and may result in permanent hearing loss. The close anatomical relationship between the middle and inner ear means that many cats with advanced middle ear infection have some degree of inner ear involvement.

Several conditions produce symptoms similar to middle ear infection and must be distinguished through diagnostic testing. Idiopathic vestibular syndrome causes sudden onset balance problems, head tilt, and nystagmus similar to middle ear infection but occurs without infection or apparent cause and typically resolves spontaneously within weeks. Brain tumors or infections affecting the central vestibular system produce similar neurological signs but arise from the brain rather than the ear. Nasopharyngeal polyps, benign inflammatory growths common in young cats, can extend into the middle ear and cause virtually identical symptoms to primary middle ear infection. Ear canal tumors, including squamous cell carcinoma and ceruminous gland tumors, can obstruct the ear canal and predispose to secondary infection while also requiring specific treatment for the underlying cancer.

Complications of middle ear infection can lead to additional serious conditions requiring management. Extension of infection to the inner ear causes otitis interna with potentially permanent vestibular and hearing damage. Facial nerve paralysis can develop when the infection affects the facial nerve as it passes through the middle ear, resulting in inability to blink, drooping lip, and drooling on the affected side. Horner's syndrome, characterized by constricted pupil, elevated third eyelid, and sunken eye appearance, occurs when sympathetic nerve fibers near the middle ear are affected. In severe cases, infection can extend through the bone to cause meningitis or brain abscess, life-threatening complications requiring intensive emergency treatment. Prevention of these complications through prompt, thorough treatment of middle ear infection underscores the importance of early diagnosis and complete treatment.
