Facial nerve paralysis in cats is a neurological condition affecting cranial nerve VII, the facial nerve, which controls the muscles of facial expression. When this nerve is damaged or dysfunctional, cats lose the ability to move the muscles on the affected side of the face, resulting in characteristic asymmetry and functional impairments. The facial nerve controls ear movement, eyelid closure, lip movement, and nostril flare on each side of the face. Paralysis can affect one side, called unilateral paralysis, or rarely both sides, called bilateral paralysis. This condition occurs in cats of all ages but is more commonly diagnosed in middle-aged to older cats.
The causes of facial nerve paralysis in cats are diverse and include ear infections extending to damage the nerve, trauma, tumors affecting the nerve or brain, inflammatory conditions, and idiopathic causes where no underlying cause can be identified. The facial nerve travels through the middle ear region, making it vulnerable to damage from severe or chronic ear infections. Tumors of the ear canal, middle ear, or brain can compress or invade the nerve. Understanding the underlying cause is important because it determines treatment approach and prognosis, though in many cases a definitive cause is not found.
Facial nerve paralysis impacts cats in several ways beyond the visible facial asymmetry. The inability to close the eyelid on the affected side puts the eye at risk for drying, ulceration, and serious complications that can threaten vision. Difficulty moving the lips affects eating and drinking, potentially causing food to fall from the affected side of the mouth. The ear on the paralyzed side may droop and cannot move normally. Some cats experience concurrent vestibular signs if the disease process also affects the inner ear, causing head tilt and balance problems. The condition affects facial expression, though cats communicate more through body language than facial expression compared to some other species.
Treatability of facial nerve paralysis varies considerably based on the underlying cause. Cases secondary to ear infection may improve significantly with treatment of the infection, though nerve recovery takes time. Idiopathic cases, like idiopathic facial paralysis in humans known as Bell's palsy, may resolve spontaneously over weeks to months. Paralysis from tumors or severe trauma carries a more guarded prognosis. Regardless of ultimate nerve recovery, protecting the eye from complications is essential during and after the paralysis period. Veterinary evaluation determines the cause and guides appropriate treatment, giving affected cats the best chance at recovery while managing complications.
