Hyperaldosteronism, also known as Conn's syndrome, is an increasingly recognized endocrine disorder in cats characterized by excessive production of the hormone aldosterone by the adrenal glands. This condition causes significant disturbances in electrolyte balance, most notably severe potassium depletion, along with systemic hypertension that can damage vital organs. Once considered rare in feline medicine, hyperaldosteronism has become more frequently diagnosed as awareness of the condition has grown and diagnostic capabilities have improved. The disorder typically affects middle-aged to older cats and can cause profound weakness and other serious clinical signs that significantly impact quality of life without appropriate treatment.
The underlying mechanism of hyperaldosteronism involves the adrenal cortex producing excessive aldosterone, the primary mineralocorticoid hormone responsible for regulating sodium and potassium balance in the body. Under normal circumstances, aldosterone acts on the kidneys to promote sodium retention and potassium excretion, helping maintain proper fluid balance and blood pressure. When aldosterone production becomes pathologically elevated, typically due to an adrenal tumor, the kidneys retain excessive sodium and water while excreting inappropriately large amounts of potassium. This creates a state of hypokalemia, or low blood potassium, which profoundly affects muscle and nerve function throughout the body. Simultaneously, the sodium and water retention drives blood pressure upward, causing hypertension.
The impact of hyperaldosteronism on affected cats manifests primarily through the consequences of severe hypokalemia and hypertension. Potassium depletion causes generalized muscle weakness that can progress to the point where cats cannot support their own body weight, creating a characteristic ventroflexion of the neck where the head drops downward. This weakness may extend to muscles throughout the body, affecting mobility and normal daily activities. Concurrent hypertension damages target organs including the eyes, heart, kidneys, and brain. Retinal changes from hypertension can cause sudden blindness. Cardiac effects include left ventricular hypertrophy as the heart works against elevated blood pressure. Kidney damage from chronic hypertension may compound any concurrent renal disease common in older cats.
Despite its serious effects, hyperaldosteronism is treatable when properly diagnosed. Surgical removal of adrenal tumors causing the condition offers the best chance of cure and is the treatment of choice when tumors are unilateral and resectable. For cats that are not surgical candidates, medical management can effectively control the condition through medications that block aldosterone's effects and potassium supplementation to correct deficiencies. Early recognition and treatment prevent progression of organ damage and restore quality of life for affected cats. The growing awareness of this condition among veterinarians means that more cats are being diagnosed earlier in the disease course when treatment is most effective, improving overall outcomes for this once under-recognized disorder.
