Aspergillosis is a fungal infection caused by Aspergillus species, most commonly Aspergillus fumigatus, that can affect cats in two primary forms: sinonasal aspergillosis affecting the nasal passages and sinuses, and disseminated aspergillosis spreading through the bloodstream to multiple organs. While relatively uncommon in cats compared to dogs, aspergillosis represents an important fungal disease with significant health implications. The organism is ubiquitous in the environment, found in soil, decaying vegetation, dust, and organic matter, meaning all cats have potential exposure, though only those with compromised defenses or overwhelming exposure develop clinical disease.
Aspergillosis develops when fungal spores are inhaled into the respiratory tract and establish infection in either the nasal passages or the lungs. In sinonasal aspergillosis, the fungus colonizes the nasal cavity and frontal sinuses, causing chronic destruction of nasal tissues and facial bones. In disseminated aspergillosis, the fungus spreads through the bloodstream from a primary respiratory site to affect multiple organs including the eyes, kidneys, liver, spleen, bones, and nervous system. Cats may be more prone to the disseminated form than dogs, particularly when underlying immunocompromise is present.
The impact of aspergillosis on affected cats varies with the form of disease but is generally serious. Sinonasal aspergillosis causes chronic nasal discharge, facial pain, and progressive tissue destruction that significantly impairs quality of life. Disseminated aspergillosis is a severe systemic infection that can affect virtually any organ, causing diverse symptoms depending on which tissues are involved. Without treatment, both forms progress and can be fatal. Even with treatment, aspergillosis is challenging to cure and requires prolonged antifungal therapy.
Treatment of aspergillosis in cats requires long-term commitment to antifungal medication, often for months or longer. Sinonasal aspergillosis may additionally benefit from topical antifungal treatment applied directly to the affected nasal passages. The prognosis depends on the form of disease, extent of involvement, immune status of the patient, and response to therapy. Many cats can achieve prolonged remission or cure with aggressive treatment, though disseminated disease carries a more guarded prognosis. Early diagnosis and prompt initiation of appropriate therapy improve outcomes significantly.
