This increased pressure places strain on the heart, blood vessels, and various target organs throughout the body, potentially leading to significant organ damage if left untreated. Feline hypertension is recognized as one of the most important cardiovascular conditions in older cats, with prevalence increasing significantly with age and in association with other common feline diseases. Unlike in humans where primary hypertension is common, cats typically develop hypertension secondary to other underlying conditions, making identification and treatment of the primary disease an essential component of management.
The causes of feline hypertension are most commonly related to other systemic diseases that affect blood pressure regulation. Chronic kidney disease represents the most common cause of hypertension in cats, with elevated blood pressure occurring in a substantial percentage of cats with renal dysfunction. Hyperthyroidism, another common condition in older cats, frequently leads to hypertension through the cardiovascular effects of excess thyroid hormone. Other less common causes include diabetes mellitus, hyperaldosteronism, pheochromocytoma, and certain cardiac conditions. In some cats, no underlying cause can be identified, a condition termed idiopathic or primary hypertension. Understanding the relationship between hypertension and these primary diseases is crucial for effective long-term management.
The impact of hypertension on affected cats can be devastating, particularly when the condition goes undiagnosed and untreated. Target organ damage affects multiple body systems, with the eyes, kidneys, heart, and brain being most vulnerable. Hypertensive retinopathy can cause sudden blindness through retinal detachment or hemorrhage, often serving as the presenting sign that prompts diagnosis. Cardiac effects include left ventricular hypertrophy and potential heart failure. Renal damage from hypertension can accelerate existing kidney disease progression. Neurological effects may include stroke-like episodes, disorientation, and seizures. The insidious nature of hypertension means significant damage may occur before obvious clinical signs develop.
Feline hypertension is a treatable condition, and with appropriate therapy, target organ damage can often be prevented or stabilized. Antihypertensive medications effectively lower blood pressure in most cats when administered consistently. Identifying and treating underlying diseases addresses the root cause of secondary hypertension. Regular blood pressure monitoring allows for dose adjustments to achieve optimal control. While some target organ damage, particularly ocular changes, may be irreversible if present at diagnosis, prompt treatment can prevent further deterioration. Early detection through routine blood pressure screening in at-risk cats provides the best opportunity to prevent the serious consequences of uncontrolled hypertension.
