Pericardial effusion is a condition in birds characterized by abnormal accumulation of fluid within the pericardial sac, the double-layered membrane that surrounds and protects the heart. Under normal circumstances, a small amount of fluid exists between the pericardial layers to reduce friction as the heart beats. When disease processes cause excess fluid to accumulate, the resulting pericardial effusion can compress the heart and interfere with its ability to fill and pump effectively. This condition may develop gradually or rapidly depending on the underlying cause, and ranges in severity from an incidental finding to a life-threatening cardiac emergency.
The development of pericardial effusion in birds occurs secondary to various disease processes rather than as a primary condition. Infections affecting the pericardium, including bacterial, viral, or fungal pathogens, can cause inflammation leading to fluid accumulation. Cardiac diseases including heart failure may result in pericardial fluid buildup. Neoplasia, or cancer, affecting the heart or pericardium commonly causes effusion in avian patients. Systemic diseases affecting blood vessels or causing generalized fluid retention may contribute. Trauma to the chest can cause bleeding into the pericardial space. Understanding the underlying cause is essential for effective treatment.
The impact of pericardial effusion on affected birds depends on the volume of fluid accumulated and how rapidly it developed. Small, slowly developing effusions may cause no clinical signs and may be discovered incidentally during imaging for other reasons. Larger effusions or those developing rapidly compress the heart within the relatively rigid pericardial sac, a condition called cardiac tamponade. This compression prevents the heart from filling properly between beats, dramatically reducing cardiac output. Affected birds may show signs of right-sided heart failure, respiratory distress, weakness, and collapse. Severe cardiac tamponade can be rapidly fatal without emergency intervention.
Treatment of pericardial effusion involves addressing both the fluid accumulation and the underlying cause. Emergency drainage of fluid through pericardiocentesis may be necessary to relieve life-threatening cardiac compression. Identifying and treating the primary disease process prevents recurrence. The prognosis depends heavily on the underlying cause, with some conditions being treatable and others carrying grave prognoses. Early recognition of symptoms and prompt veterinary care provide the best outcomes. Birds at risk due to cardiac disease, systemic illness, or neoplasia should be monitored for signs of pericardial involvement.
