Nystatin is a polyene antifungal antibiotic originally derived from Streptomyces noursei, a soil bacterium discovered in the 1950s. This medication has remained a valuable tool in both human and veterinary medicine for treating superficial yeast and fungal infections due to its excellent efficacy against Candida species and related organisms. In canine medicine, nystatin is primarily used topically or in combination ear medications to address yeast infections of the skin, ear canal, and mucosal surfaces. While newer antifungal agents have been developed over the decades since nystatin's discovery, it continues to be prescribed due to its targeted activity, minimal absorption, and excellent safety profile when applied to external body surfaces.
The mechanism of action of nystatin involves binding to ergosterol, a sterol component unique to fungal cell membranes. When nystatin molecules bind to ergosterol, they create pores or channels in the fungal cell membrane that allow essential cellular contents including potassium and other ions to leak out of the cell. This membrane disruption leads to rapid fungal cell death. The specificity of nystatin for ergosterol, which is not found in mammalian cell membranes, explains its selectivity for fungal cells and its excellent safety when applied topically. At therapeutic concentrations, nystatin has fungistatic and fungicidal activity against susceptible organisms, with the concentration and contact time determining whether organisms are inhibited or killed.
Nystatin is available in multiple formulations for veterinary use, most commonly as creams, ointments, and powders for topical application, and as a component of combination otic preparations for treating ear infections. Oral nystatin suspension is occasionally prescribed for gastrointestinal yeast overgrowth, though this use is less common in dogs than topical applications. Many veterinary ear medications combine nystatin with antibacterial agents and corticosteroids to address the multiple components typically present in canine otitis, including bacteria, yeast, and inflammation. The powder formulation is particularly useful for treating moist, intertriginous areas like skin folds where its drying properties complement its antifungal activity.
When used according to veterinary instructions, nystatin has an excellent safety profile for dogs across all ages and breeds. The medication is not significantly absorbed through intact skin or the gastrointestinal tract, which minimizes systemic effects while providing effective local treatment. This minimal absorption makes nystatin one of the safest antifungal options for dogs with liver or kidney compromise who cannot safely receive systemic antifungal medications.
