Neomycin is an aminoglycoside antibiotic used in veterinary medicine primarily for topical applications and oral gastrointestinal therapy in dogs. Unlike other aminoglycosides that are primarily administered by injection, neomycin's therapeutic niche lies in its use as a topical agent for skin, ear, and eye infections, as well as its oral use for reducing intestinal bacteria in specific medical conditions. The drug has been a staple of veterinary medicine for decades, valued for its broad-spectrum activity against gram-negative bacteria and its availability in numerous combination products designed for localized treatment of common canine infections.
The mechanism of action of neomycin involves irreversible binding to the 30S ribosomal subunit of susceptible bacteria, disrupting protein synthesis and causing bacterial cell death. This bactericidal activity makes neomycin effective at eliminating bacteria rather than merely inhibiting their growth. The drug demonstrates excellent activity against many aerobic gram-negative organisms including Escherichia coli, Proteus species, Klebsiella species, and Enterobacter species. Neomycin also shows effectiveness against certain gram-positive bacteria, particularly Staphylococcus species that commonly cause skin infections in dogs. However, the drug has limited activity against Pseudomonas aeruginosa, which distinguishes it from some other aminoglycosides.
Neomycin is available in multiple formulations suited to its various therapeutic applications. Topical preparations, often combined with other antibiotics such as polymyxin B and bacitracin, treat superficial skin infections and prevent wound infection. Ophthalmic solutions containing neomycin address bacterial conjunctivitis and other external eye infections. Otic preparations, frequently combined with antifungal and anti-inflammatory agents, treat bacterial components of ear infections. Oral neomycin tablets are used for reducing intestinal bacterial populations, particularly in dogs with hepatic encephalopathy where ammonia-producing bacteria must be controlled. This oral use takes advantage of neomycin's poor absorption from the gastrointestinal tract, allowing the drug to act locally in the intestines.
While topical neomycin carries relatively low systemic toxicity risk, understanding proper use remains important for safe and effective treatment. Systemic absorption can occur through damaged skin, prolonged use on large areas, or through oral administration in patients with compromised intestinal barriers. The nephrotoxicity and ototoxicity concerns associated with systemic aminoglycoside exposure remain relevant when absorption is possible. Allergic contact dermatitis is a recognized concern with topical neomycin use, and treatment should be discontinued if signs of hypersensitivity develop. Veterinary supervision ensures appropriate product selection, proper application technique, and monitoring for adverse effects throughout the treatment course.
