Hydralazine is a direct-acting arteriolar vasodilator that has been used in veterinary cardiology for several decades to reduce afterload and manage hypertension in dogs with various cardiovascular conditions. As one of the original vasodilator medications adapted from human medicine for veterinary use, hydralazine has established clinical efficacy for reducing the resistance against which the heart must pump, thereby improving cardiac output in dogs with heart failure. While newer medications have emerged that may offer advantages in some situations, hydralazine remains a valuable option, particularly for acute afterload reduction and in situations where other vasodilators have proven inadequate or are contraindicated. The medication is classified as a human pharmaceutical used off-label in veterinary patients.
The mechanism of action of hydralazine involves direct relaxation of vascular smooth muscle in arterioles, the small blood vessels that control peripheral vascular resistance. Unlike calcium channel blockers or ACE inhibitors that work through receptor-mediated mechanisms, hydralazine acts directly on the smooth muscle cells to cause vasodilation. The exact molecular mechanism is not completely understood but appears to involve interference with calcium-mediated contractile mechanisms in vascular smooth muscle. Importantly, hydralazine has minimal direct effect on venous capacitance vessels, meaning it primarily reduces arterial resistance (afterload) rather than affecting preload. This selective arterial action makes it particularly useful for dogs with mitral valve regurgitation, where reducing the resistance to forward flow helps decrease the backward leak through the incompetent valve.
Hydralazine is available in tablet form for oral administration, with common strengths including 10 mg, 25 mg, and 50 mg tablets. An injectable formulation is available for hospital use when rapid vasodilation is needed, such as in hypertensive emergencies. For oral maintenance therapy, tablets are typically administered two to three times daily due to the medication's relatively short duration of action. This multiple daily dosing requirement distinguishes hydralazine from some newer vasodilators that can be given once daily. Compounded formulations may be available for dogs requiring non-standard doses, particularly very small patients where standard tablet sizes are difficult to divide accurately.
The safety profile of hydralazine requires careful consideration and close monitoring, particularly during initiation of therapy and dose adjustment periods. The medication can cause significant and sometimes unpredictable drops in blood pressure, making it essential to start at low doses and titrate carefully. Reflex tachycardia, a compensatory increase in heart rate in response to blood pressure reduction, is a common effect that may require concurrent use of beta-blockers or digoxin to control. The need for multiple daily doses and careful monitoring make hydralazine more management-intensive than some alternative vasodilators, but its potency and relatively low cost keep it relevant in veterinary cardiovascular medicine. Regular veterinary follow-up is essential for dogs receiving hydralazine therapy.
