Vidarabine, also known by its chemical name adenine arabinoside or Ara-A, is an antiviral medication used topically for the treatment of ocular infections caused by feline herpesvirus type 1 (FHV-1) in cats. Originally developed from cultures of Streptomyces antibioticus and approved for human herpesviral infections, vidarabine has found important application in veterinary ophthalmology as an off-label treatment for herpetic eye disease in cats. The medication is particularly valued for its generally excellent tolerability in feline patients, often being better tolerated than some alternative topical antivirals, making it a useful option for cats requiring antiviral therapy for ocular herpes.
Vidarabine works through a mechanism distinct from the thymidine analogs like idoxuridine and trifluridine. As an adenosine analog, vidarabine is phosphorylated by cellular enzymes to its active triphosphate form, vidarabine triphosphate. This active compound interferes with viral DNA synthesis by inhibiting viral DNA polymerase, thereby preventing the virus from replicating within infected cells. This different mechanism of action compared to thymidine analogs means that vidarabine may be effective in cases where other antivirals have not achieved adequate response, as resistance patterns may differ between drug classes.
The medication is available as a 3% ophthalmic ointment formulation. The ointment vehicle offers certain practical advantages, including longer contact time with the ocular surface compared to solution formulations and a lubricating effect that may be soothing for inflamed eyes. Vidarabine is no longer commercially manufactured as a ready-to-use product in many regions but can be obtained through compounding pharmacies that prepare it specifically for veterinary use. The ointment is applied directly to the affected eye, typically five to six times daily during active treatment, following the general principle that applies to all virostatic antivirals.
While vidarabine is too toxic for systemic administration due to its effects on host DNA synthesis, the topical ophthalmic route results in minimal systemic absorption, allowing safe use for ocular herpetic disease. The medication is considered to have moderate antiviral potency against FHV-1 in laboratory studies, falling between trifluridine (more potent) and acyclovir (less potent) in comparative efficacy assessments. However, clinical response depends on multiple factors beyond in vitro potency, and vidarabine remains a valuable option in the veterinary antiviral armamentarium. Veterinary supervision is important for appropriate diagnosis, treatment selection, and monitoring throughout therapy.
