Eosinophilic keratitis is a distinctive inflammatory condition of the feline cornea characterized by the infiltration of eosinophils and other inflammatory cells into the corneal tissue. Eosinophils are white blood cells typically associated with allergic reactions and parasitic infections, and their presence in the cornea creates a unique appearance that distinguishes this condition from other types of keratitis. The condition typically manifests as raised, white to pink, chalky or caseous plaques on the corneal surface, often beginning at the limbus, the junction between the cornea and sclera, and spreading centrally. Eosinophilic keratitis can affect one or both eyes and is considered relatively common among feline corneal diseases.
The exact cause of eosinophilic keratitis remains incompletely understood, but a strong association with feline herpesvirus type 1 has been established. Many cats with eosinophilic keratitis test positive for herpesvirus, suggesting that viral infection triggers or perpetuates the immune-mediated inflammatory response. However, not all affected cats have detectable herpesvirus, indicating that other factors may also initiate the condition. The disease appears to represent an aberrant immune response, possibly an allergic-type reaction, to antigenic stimulation within the cornea. Some researchers consider it part of the feline eosinophilic syndrome that can affect multiple tissues.
The impact of eosinophilic keratitis on affected cats ranges from mild visual impairment to significant vision loss depending on the extent of corneal involvement. The raised plaques are opaque, and when they cover the central visual axis, they substantially block light transmission. Left untreated, the condition tends to progress, with plaques expanding to cover increasingly large portions of the cornea. The inflammatory process can cause discomfort, though affected cats may show surprisingly few pain signs compared to cats with corneal ulcers. Chronic cases may develop permanent corneal scarring and vascularization even after the active disease is controlled.
Eosinophilic keratitis is manageable with appropriate immunosuppressive and anti-inflammatory therapy, though it is often a chronic condition requiring long-term treatment. Most cats respond well initially to topical corticosteroids or other immunomodulatory medications, with plaques resolving over weeks to months. However, recurrence is common when treatment is discontinued, and many cats require maintenance therapy indefinitely. Early diagnosis and treatment minimize permanent corneal damage. With dedicated owner commitment to ongoing management, most cats with eosinophilic keratitis maintain comfortable, functional vision throughout their lives.
