Neomycin is an aminoglycoside antibiotic with a distinctive role in feline medicine, primarily used for its local effects in the gastrointestinal tract or for topical application rather than systemic therapy. Unlike other aminoglycosides such as gentamicin and amikacin that are administered by injection for systemic infections, neomycin is most commonly given orally to reduce bacterial populations in the intestine or applied topically to treat localized infections of the skin, ears, and eyes. This unique application pattern makes neomycin a valuable therapeutic tool while avoiding some of the serious systemic toxicity concerns associated with injectable aminoglycosides.
The mechanism of action of neomycin is identical to other aminoglycosides, involving irreversible binding to the bacterial 30S ribosomal subunit, which disrupts protein synthesis and results in bacterial cell death. Neomycin demonstrates bactericidal activity against many gram-negative bacteria including Escherichia coli and various enteric pathogens, as well as some gram-positive organisms. When administered orally, neomycin is very poorly absorbed from the gastrointestinal tract, which means the drug remains largely within the intestinal lumen where it can exert local antibacterial effects without significant systemic exposure. This poor absorption is actually advantageous for its primary clinical applications.
Neomycin is available in multiple formulations designed for different routes of administration in cats. Oral tablets and solutions are used to reduce intestinal bacterial populations in conditions such as hepatic encephalopathy. Topical preparations, often combined with other antibiotics and anti-inflammatory agents, are used for skin infections, wound treatment, and prevention of infection in minor injuries. Ophthalmic formulations treat bacterial conjunctivitis and other eye infections, while otic preparations address bacterial ear infections. Many of these topical products are combination medications that include neomycin alongside other active ingredients to provide broad-spectrum antimicrobial coverage.
Although neomycin is considerably safer than systemically administered aminoglycosides due to its poor gastrointestinal absorption, it is not without risk. Prolonged oral administration or use in patients with compromised intestinal barriers can result in sufficient systemic absorption to cause nephrotoxicity and ototoxicity similar to injectable aminoglycosides. Topical application to large areas of damaged skin or mucous membranes can similarly lead to significant absorption. Additionally, oral neomycin can cause gastrointestinal disturbances including diarrhea and malabsorption. Veterinary supervision remains essential to ensure appropriate use, proper dosing, and monitoring for adverse effects throughout treatment.
