Griseofulvin, available under the brand name Fulvicin among others, is an oral antifungal medication that has been used for decades to treat dermatophytosis, commonly known as ringworm, in cats. Derived from Penicillium griseofulvum, this medication was one of the first effective oral treatments for fungal skin infections and remains a viable option for treating dermatophyte infections in feline patients. While newer antifungal medications have become available with potentially fewer side effects and shorter treatment durations, griseofulvin continues to be used in veterinary medicine due to its proven efficacy, familiarity among practitioners, and cost-effectiveness.
Griseofulvin works through a unique mechanism of action that distinguishes it from other antifungal medications. The drug interferes with fungal cell division by disrupting the spindle apparatus during mitosis, preventing the fungal cells from reproducing normally. Griseofulvin binds to newly formed keratin in the skin, hair, and nails, making these tissues resistant to fungal invasion as they grow and replace infected structures. This mechanism means that the drug does not kill existing fungal elements but rather prevents new infection of growing tissues, requiring treatment to continue until all infected material has been replaced with healthy, drug-containing keratin.
Griseofulvin is available in different particle size formulations, including microsize and ultramicrosize preparations, which differ in their absorption characteristics. The ultramicrosize formulations have improved bioavailability and allow for lower doses to achieve equivalent blood levels, though microsize preparations are also effective when dosed appropriately. The medication is typically administered orally once or twice daily with food, as fat enhances absorption of the drug. Treatment duration for dermatophytosis is typically extended, often requiring six weeks or longer, continuing until fungal cultures confirm eradication of the infection.
While griseofulvin has a long track record of use in veterinary medicine, it does carry some important safety considerations. The medication is teratogenic and absolutely contraindicated in pregnant cats. Bone marrow suppression, particularly in cats infected with feline immunodeficiency virus, is a serious potential adverse effect. Veterinary supervision is essential for appropriate use of griseofulvin, including proper diagnosis of dermatophytosis, selection of appropriate candidates for therapy, accurate dosing, and monitoring for adverse effects throughout the treatment course.
