The mechanism of action of glargine insulin involves its unique molecular structure that provides variable solubility at different pH levels. Glargine is supplied and stored at an acidic pH of 4.0, at which it remains soluble. Upon subcutaneous injection into the neutral pH environment of body tissues, the insulin forms microprecipitates that slowly dissolve and release small amounts of insulin over an extended period. This results in a relatively flat pharmacodynamic profile often described as having no pronounced peak, though cats do experience a nadir in blood glucose concentrations during treatment. The duration of action in cats typically ranges from twelve to sixteen hours or longer, making it suitable for twice-daily administration in most patients.
Glargine insulin is available exclusively in injectable form at a concentration of 100 units per milliliter. The medication comes in both vials and prefilled insulin pens, with various brands including the original Lantus, Basaglar, Semglee, and generic glargine. For cats, the insulin must be administered using U-100 syringes to ensure accurate dosing. The typical administration protocol involves subcutaneous injections given twice daily at twelve-hour intervals, ideally after meals. The small doses required for most cats, typically one to two units per injection, can make accurate measurement challenging, and U-100 syringes with half-unit markings are often recommended to improve dosing precision.
The safety profile of glargine insulin in cats is generally favorable when used appropriately under veterinary supervision. The primary risk associated with any insulin therapy is hypoglycemia, which can occur if the insulin dose is too high or if the cat does not eat adequately before or after injection. Proper monitoring of blood glucose levels is essential throughout treatment. Glargine should never be diluted or mixed with other insulins, as this interferes with its unique crystallization properties that provide extended duration.
