Neomycin is an aminoglycoside antibiotic with a long history of use in veterinary medicine, employed primarily for oral treatment of gastrointestinal infections and topical treatment of skin, wound, and eye infections in avian patients. Unlike other aminoglycosides that are administered parenterally for systemic infections, neomycin is considered too toxic for injection and is used exclusively via oral or topical routes in clinical practice. This limitation to local applications actually provides certain advantages, as the poor gastrointestinal absorption of neomycin means that oral administration delivers high drug concentrations to the intestinal tract while minimizing systemic exposure and associated toxicity risks.
The mechanism of action of neomycin follows the pattern common to all aminoglycoside antibiotics, involving irreversible binding to the 30S ribosomal subunit of bacterial cells. This binding interferes with bacterial protein synthesis, causing the production of abnormal proteins and ultimately leading to bacterial cell death. Neomycin exhibits bactericidal activity against a range of gram-negative bacteria including Escherichia coli, Klebsiella species, and Enterobacter organisms. The drug also demonstrates activity against some gram-positive bacteria, particularly certain Staphylococcus species. However, bacterial resistance to neomycin has become increasingly common over the decades of its use, and susceptibility testing is advisable when treating identified bacterial infections.
Neomycin is available in several formulations suited to its oral and topical applications. Oral preparations include solutions and tablets used for treating bacterial enteritis and for gastrointestinal decontamination protocols. Topical formulations include creams, ointments, and ophthalmic preparations, often in combination with other antibiotics or anti-inflammatory agents. These multi-agent topical products can provide broader spectrum coverage and additional therapeutic benefits for skin and wound infections. The versatility of available formulations allows neomycin to be adapted to various clinical situations where local antibiotic delivery is appropriate.
While neomycin's restriction to oral and topical use reduces the risk of systemic toxicity compared to injectable aminoglycosides, the medication is not without potential adverse effects. Oral neomycin can disrupt normal intestinal flora and cause gastrointestinal disturbances. Prolonged oral use can result in some systemic absorption, particularly if the intestinal lining is damaged, potentially leading to nephrotoxicity or ototoxicity. Topical application to large areas of damaged skin can also result in systemic absorption. Despite these considerations, neomycin remains a useful tool for specific indications in avian medicine when used appropriately under veterinary supervision with awareness of its limitations and potential risks.
