Lymphocytic choriomeningitis (LCM) is a viral disease caused by the lymphocytic choriomeningitis virus (LCMV), an arenavirus that primarily affects rodents and can cause serious neurological disease. The name describes the characteristic inflammation of the meninges (the membranes covering the brain) with infiltration of lymphocytes (a type of white blood cell). While the common house mouse is the primary natural reservoir for this virus, pet rodents including hamsters, guinea pigs, and other species can become infected and develop disease. Importantly, LCMV is zoonotic, meaning it can be transmitted from infected rodents to humans, where it can cause flu-like illness or, more seriously, meningitis or encephalitis.
This condition has significant implications for both animal and human health. In rodents, LCM can range from asymptomatic carrier status, particularly in mice infected from birth, to severe fatal disease in animals infected later in life or in susceptible species like hamsters. The virus establishes persistent infection in mice infected in utero or shortly after birth, who then shed virus throughout their lives without showing illness. In contrast, hamsters and guinea pigs often develop clinical disease when infected. The potential for human transmission makes this disease a significant public health concern and has led to outbreaks traced to pet store hamsters and laboratory mouse colonies.
The impact of LCM on affected small mammals varies significantly based on species, age at infection, and immune status. Mice that carry the virus from birth typically appear healthy but are lifelong shedders of infectious virus. Hamsters infected as juveniles or adults often develop severe illness including neurological symptoms, wasting, and death. Guinea pigs may develop acute fatal disease. The clinical presentation can mimic other conditions, making diagnosis challenging without specific testing. The neurological involvement can cause devastating symptoms affecting quality of life and survival.
There is no specific antiviral treatment for LCM in small mammals, making prevention the primary focus of management. Supportive care can be provided for affected animals, but the prognosis for clinically ill animals is guarded to poor. The zoonotic potential of this virus means that diagnosis has implications beyond the affected animal, including considerations for human household members, particularly pregnant women and immunocompromised individuals who are at highest risk for severe human disease. Care from an exotic veterinarian is essential for proper diagnosis and guidance on both animal treatment and human exposure considerations.
