When a hemipenis fails to retract, the exposed tissue rapidly becomes swollen, dried, and damaged, creating an urgent situation requiring immediate intervention. Without prompt treatment, tissue death leads to loss of the hemipenis, potential systemic infection, and death.
Leopard geckos are among the reptile species most commonly presented with hemipene prolapse, likely reflecting their popularity in captivity combined with husbandry and breeding practices that predispose to this condition. Male leopard geckos possess two hemipenes, one on each side, either of which may prolapse independently. The hemipenes serve exclusively reproductive functions and are not involved in urination, which occurs through a separate urinary opening. This anatomical arrangement means that while hemipene loss affects breeding ability, it does not directly affect urinary function, though complications can involve adjacent structures.
The impact of hemipene prolapse on an affected gecko ranges from temporary discomfort with full recovery to permanent reproductive damage, systemic illness, or death, depending on how quickly the condition is recognized and treated. Prolapsed tissue begins deteriorating within hours of exposure, with swelling impeding spontaneous retraction and desiccation causing tissue damage. Bacteria can rapidly colonize the compromised tissue, leading to infection. Necrotic (dead) tissue can seed infection throughout the body. The prognosis is directly related to the duration of prolapse before treatment begins.
