This condition causes squamous metaplasia, a pathological process in which the normal columnar or cuboidal epithelial cells lining various body surfaces transform into abnormal, keratinized squamous cells. When this process affects the conjunctiva, eyelids, and associated glands of the eye, it results in the characteristic ocular signs that often bring affected reptiles to veterinary attention. The periocular manifestations of hypovitaminosis A represent just one component of what is typically a systemic nutritional disease affecting epithelial tissues throughout the body.
Vitamin A deficiency is most commonly diagnosed in chelonians, particularly aquatic and semi-aquatic turtles such as box turtles and red-eared sliders, where it has reached epidemic proportions due to widespread feeding of inappropriate diets. However, this condition also affects tortoises, many lizard species particularly those fed monotonous insect-only diets, and occasionally other reptiles maintained on nutritionally deficient diets. The prevalence of hypovitaminosis A directly reflects the quality of captive husbandry, with the condition being essentially non-existent in reptiles fed properly formulated, species-appropriate diets rich in vitamin A precursors and preformed vitamin A.
The impact of hypovitaminosis A on ocular health is profound and progressive. Early changes may be subtle, but as the condition advances, affected reptiles develop increasingly severe conjunctival and eyelid pathology. The squamous metaplasia affects the conjunctival epithelium and the accessory glands responsible for tear production, leading to reduced tear output, accumulation of cellular debris, and susceptibility to secondary bacterial infection. Swelling of the eyelids becomes pronounced, often causing the eyes to appear completely closed or bulging with retained debris. Without treatment, affected reptiles may experience permanent damage to ocular structures and potentially total vision loss.
With appropriate veterinary intervention and dietary correction, hypovitaminosis A is a treatable condition, though the degree of recovery depends on the severity and duration of deficiency before treatment begins. Mild cases caught early may resolve completely with dietary modification alone, while advanced cases require injectable vitamin A supplementation, topical treatments, and sometimes surgical debridement of accumulated debris. The key to successful treatment lies in addressing both the immediate periocular pathology and the underlying nutritional deficiency through permanent dietary improvements. Recurrence is common if dietary changes are not maintained, emphasizing the importance of owner education regarding proper reptile nutrition.
