Cataracts, whether congenital, developmental, or E. cuniculi-induced, often precede phacoclastic uveitis as the lens degenerates. Secondary glaucoma frequently develops as a complication when inflammation blocks aqueous humor drainage. Gastrointestinal stasis can develop secondary to the pain and stress of phacoclastic uveitis, making monitoring of appetite and fecal output essential throughout treatment.

Conditions with similar symptoms to phacoclastic uveitis include other forms of anterior uveitis from various causes including bacterial infection, trauma without lens involvement, or immune-mediated disease. Glaucoma causes a painful, often enlarged eye that may appear similar, though intraocular pressure measurement differentiates the conditions. Corneal ulceration causes pain, discharge, and squinting and may occur concurrently with or be mistaken for uveitis. Conjunctivitis causes red, irritated eyes but typically lacks the deeper inflammation of uveitis. Anterior lens luxation, where the lens displaces forward, can cause similar signs and may complicate existing uveitis. Accurate diagnosis through thorough ophthalmic examination is essential because treatments differ significantly.

Potential complications of phacoclastic uveitis include permanent vision loss in the affected eye, which occurs commonly despite treatment. Synechiae formation, where the iris adheres to the lens or cornea, can cause pupil abnormalities and contribute to glaucoma development. Secondary glaucoma from blocked aqueous drainage causes additional pain and potential globe enlargement. Phthisis bulbi, where the eye shrinks and becomes non-functional, may occur with chronic inflammation. Hyphema, or blood in the anterior chamber, can develop in severe cases. Septic endophthalmitis from secondary bacterial infection is a serious complication requiring aggressive treatment. Prevention of these complications through prompt diagnosis and aggressive treatment is essential for the best possible outcome.
