Encephalitozoon cuniculi, commonly abbreviated as E. cuniculi or EC, is a microsporidian parasite that represents one of the most significant parasitic diseases affecting domestic rabbits worldwide. This obligate intracellular organism is classified as a primitive fungus-related microorganism that invades and replicates within the host's cells, primarily targeting the central nervous system, kidneys, and eyes. Studies indicate that anywhere from 25% to 80% of pet rabbits in the United States test seropositive for E. cuniculi, meaning they have been exposed to the parasite at some point in their lives, though many of these rabbits remain asymptomatic throughout their entire lifespan.
The parasite spreads primarily through the ingestion of spores shed in the urine of infected rabbits. Once ingested, E. cuniculi spores travel through the bloodstream and establish themselves in target organs, where they replicate inside host cells until the cells rupture, releasing more spores and causing inflammation and tissue damage. This cycle of cellular invasion, replication, and rupture is responsible for the clinical signs observed in symptomatic rabbits. Vertical transmission from mother to offspring can also occur during pregnancy, which may explain why some rabbits develop ocular manifestations of the disease at a young age.
The impact of E. cuniculi on rabbit health can range from completely asymptomatic infection to severe, life-threatening disease. When clinical signs do develop, they typically reflect damage to the nervous system, kidneys, or eyes, and may include head tilt, loss of balance, hind limb weakness, urinary problems, and lens abnormalities. The severity of clinical disease does not necessarily correlate with the presence of active parasites but rather with the inflammation caused by the immune response to ruptured host cells. This means that rabbits can experience significant symptoms even after the active infection has been controlled, as the inflammatory damage persists.
Treatment for E. cuniculi involves antiparasitic medication, most commonly fenbendazole, administered over a course of 28 days. While this treatment effectively reduces spore proliferation and prevents further tissue invasion, it cannot reverse damage that has already occurred to the nervous system, kidneys, or eyes. Early detection and intervention are therefore critical to achieving the best possible outcomes, though the challenge lies in the fact that rabbits instinctively hide signs of illness as a survival mechanism.
