Middle ear infection, medically termed otitis media, is a serious bacterial infection affecting the middle ear cavity and tympanic bulla in rabbits. The middle ear is the air-filled chamber located behind the eardrum that contains the small bones responsible for transmitting sound vibrations to the inner ear. In rabbits, the middle ear is enclosed within a bony structure called the tympanic bulla. When bacteria invade this protected space, the resulting infection causes severe pain, neurological symptoms, and potentially life-threatening complications. Middle ear infections are common in rabbits, particularly in lop-eared breeds, and represent one of the more challenging ear conditions to treat due to the difficulty of delivering medications to the enclosed middle ear space.
Middle ear infections in rabbits most commonly develop as an extension of outer ear canal infections that progress past the eardrum, either through perforation or along the tissues. The bacteria Pasteurella multocida is the most frequent causative organism, though other bacteria including Staphylococcus, Streptococcus, Pseudomonas, and Bordetella may also be involved. In some cases, bacteria reach the middle ear through the Eustachian tube from respiratory infections. Rabbits with chronic outer ear disease, particularly lop-eared breeds with their predisposed ear anatomy, face the highest risk of developing middle ear involvement. Once established, middle ear infections create a protected reservoir of infection that is extremely difficult to eliminate with systemic antibiotics alone.
The impact of middle ear infection on rabbit health extends beyond the ear itself due to the close proximity of critical structures. The vestibular system, responsible for balance and spatial orientation, lies adjacent to the middle ear, and inflammation or infection can affect these structures causing characteristic vestibular signs including head tilt, circling, and nystagmus. The facial nerve passes near the middle ear and may be affected, causing facial paralysis. Severe or untreated infections can spread to the inner ear causing permanent deafness, or even to the brain causing life-threatening meningitis or brain abscess. The pain associated with middle ear infection frequently causes rabbits to stop eating, which can trigger GI stasis, a condition that can become fatal within twenty-four to forty-eight hours. Because rabbits instinctively hide signs of illness, middle ear infections may become quite advanced before owners recognize the severity of the problem.
Middle ear infection in rabbits requires aggressive treatment and carries a guarded to fair prognosis depending on severity and promptness of intervention. Treatment typically involves prolonged courses of systemic antibiotics, ideally chosen based on culture and sensitivity testing. Surgical intervention may be necessary for cases that do not respond to medical management or when abscessation is present. A rabbit-savvy veterinarian is essential for proper diagnosis and treatment, as the specialized imaging, surgical techniques, and anesthetic protocols required are beyond the scope of general small animal practice. Early detection significantly improves outcomes, but the subtle early signs of middle ear disease make this challenging. With appropriate treatment, many rabbits can recover from middle ear infection, though some may retain permanent vestibular deficits or hearing loss even after infection is resolved.
