Inner ear infection, medically known as otitis interna, is a serious bacterial infection affecting the deepest structures of the rabbit's ear, including the cochlea (responsible for hearing), the vestibular apparatus (responsible for balance), and the semicircular canals (detecting rotational movement). This condition represents the most severe form of ear infection and typically develops as a progression from middle ear infection (otitis media) when bacteria penetrate the delicate membranes separating the middle and inner ear. Inner ear infection causes profound effects on balance and coordination, commonly resulting in head tilt, nystagmus (involuntary eye movement), and severe disorientation that can leave affected rabbits unable to walk or stand normally.
The most common causative organism in rabbit inner ear infections is Pasteurella multocida, a bacterium that normally colonizes the respiratory tract of many rabbits without causing problems. Under conditions of stress, immune suppression, or local tissue damage, Pasteurella can become pathogenic and spread to the middle ear through the Eustachian tube, then progress to the inner ear. Other bacteria including Staphylococcus, Pseudomonas, and Bordetella may also be involved, sometimes in mixed infections. The warm, enclosed environment of the rabbit's middle ear, particularly in lop breeds, provides conditions favorable for bacterial growth and persistence.
The impact of inner ear infection on rabbit health is significant and can be life-threatening if not promptly and aggressively treated. The vestibular dysfunction causes severe nausea and disorientation that typically causes affected rabbits to stop eating, rapidly leading to secondary gastrointestinal stasis. The dramatic neurological signs, including head tilt, falling, rolling, and inability to walk, can be terrifying for both rabbits and their owners. Hearing loss often accompanies the vestibular symptoms, though this is difficult to assess definitively in rabbits. Without treatment, the infection can spread to the brain (meningoencephalitis), causing fatal septic complications. Even with appropriate treatment, permanent vestibular damage and residual head tilt are common.
Treatment of inner ear infection requires extended antibiotic therapy, typically for four to eight weeks or longer, along with intensive supportive care to maintain nutrition and prevent secondary complications. A rabbit-savvy veterinarian is essential for appropriate diagnosis and treatment, as the choice of antibiotics is critical. Many common antibiotics are fatal to rabbits when given orally, and only experienced rabbit practitioners understand the safe options and appropriate treatment protocols. With aggressive treatment and dedicated nursing care, many rabbits can recover from inner ear infection, though residual effects are common. Prevention focuses on prompt treatment of external and middle ear infections before they can progress to inner ear involvement.
