Immune-mediated thrombocytopenia, commonly abbreviated as ITP or IMT, is a serious autoimmune condition in which a rabbit's own immune system mistakenly attacks and destroys its platelets. Platelets are essential blood cells produced in the bone marrow that play a critical role in blood clotting and wound healing. When the immune system targets these cells for destruction, the platelet count drops dangerously low, leaving the rabbit vulnerable to spontaneous bleeding and hemorrhage. While relatively uncommon in rabbits compared to some other species, thrombocytopenia from various causes is a recognized clinical finding in rabbit medicine, with immune-mediated destruction being one potential etiology that requires prompt veterinary attention.
The condition develops when the immune system produces antibodies that bind to the surface of platelets, marking them for destruction by phagocytic cells primarily located in the spleen. This process can occur as a primary idiopathic condition where no underlying trigger is identified, or as a secondary response to infections, medications, vaccinations, or underlying diseases such as cancer. In rabbits, thrombocytopenia is commonly associated with sepsis, disseminated intravascular coagulation, uterine disease including uterine adenocarcinoma, liver lobe torsion, and viral infections such as rabbit hemorrhagic disease. Entire female rabbits appear to have increased predisposition to thrombocytopenia due to excessive consumption of platelets associated with uterine disorders.
The impact of immune-mediated thrombocytopenia on a rabbit's health can be profound and potentially life-threatening. Normal platelet counts in rabbits range from 250,000 to 650,000 per microliter of blood. When counts fall significantly below this range, the rabbit loses its ability to form proper blood clots, leading to spontaneous bleeding that can manifest as petechiae (tiny red spots on the skin and mucous membranes), ecchymoses (larger bruises), epistaxis (nosebleeds), blood in the urine or feces, or internal hemorrhaging. Because rabbits are prey animals that instinctively hide signs of illness, owners may not notice symptoms until the condition has become severe and the rabbit is experiencing significant bleeding complications.
With early detection and appropriate treatment, many rabbits with immune-mediated thrombocytopenia can achieve remission, though the prognosis varies depending on the underlying cause and the rabbit's response to therapy. Treatment primarily involves immunosuppressive medications to halt the immune system's attack on platelets, along with supportive care to manage bleeding and maintain overall health. Finding a rabbit-savvy veterinarian with experience in exotic animal medicine is essential, as the diagnosis and treatment of immune-mediated conditions in rabbits requires specialized knowledge that differs significantly from approaches used in dogs and cats. Early intervention dramatically improves outcomes, making awareness of this condition and its warning signs critically important for rabbit owners.
