Endocarditis is an infection of the inner lining of the heart, particularly affecting the heart valves, that occurs when bacteria or other microorganisms colonize the cardiac tissues. In rabbits, this serious condition typically develops when bacteria from an infection elsewhere in the body enter the bloodstream and settle on the heart valves. While endocarditis is relatively uncommon in rabbits compared to some other cardiac conditions, it represents a significant health threat due to the damage it causes to heart valve function and its potential to spread infection to other organs. Recognition of this condition requires a high index of suspicion since its symptoms often overlap with other illnesses.
The causes of endocarditis in rabbits are primarily bacterial, with organisms entering the bloodstream from infections at other body sites. Dental disease, which is extremely common in rabbits, represents a major potential source of bacteria that can reach the heart. Chronic abscesses anywhere in the body, skin infections, respiratory infections, and urinary tract infections can all serve as sources of bloodstream bacteria. Once in the bloodstream, bacteria can adhere to heart valves, particularly if the valves have any preexisting damage or abnormality that creates turbulent blood flow and allows bacteria to attach more easily.
The impact of endocarditis on affected rabbits can be devastating. As bacteria colonize and grow on the heart valves, they form vegetations that interfere with normal valve function. This can cause valves to leak (regurgitation) or, less commonly, to become obstructed. The resulting valve dysfunction forces the heart to work harder, potentially leading to heart failure over time. Additionally, pieces of infected vegetation can break off and travel through the bloodstream to lodge in other organs, causing secondary infections in the kidneys, brain, lungs, or other tissues. The infection also causes systemic illness with fever, loss of appetite, and general malaise.
Treatment of endocarditis requires aggressive, prolonged antibiotic therapy using rabbit-safe antibiotics, combined with supportive care and treatment of the underlying source of infection. Early diagnosis and treatment are essential for the best outcomes, though even with aggressive therapy the prognosis is guarded. Care must be provided by a rabbit-savvy veterinarian familiar with the antibiotics that are safe for rabbits, as many commonly used antibiotics in other species are toxic to rabbits and can be fatal. The condition demonstrates why prompt treatment of infections and excellent dental care are important aspects of rabbit health management.
