Hepatic coccidiosis is a parasitic disease affecting the liver and biliary system of rabbits, caused by the protozoan parasite Eimeria stiedae. Unlike the more common intestinal forms of coccidiosis in rabbits, this hepatic variant specifically targets the liver and bile ducts, causing progressive liver damage that can range from mild subclinical infection to severe, life-threatening hepatic failure. Eimeria stiedae is highly host-specific, affecting only rabbits and hares, and is distinct from the multiple Eimeria species that cause intestinal coccidiosis. Hepatic coccidiosis represents a significant health concern particularly in young rabbits, rabbit breeding operations, and situations where sanitation may be suboptimal. The disease has been recognized in domestic rabbits worldwide and remains an important cause of morbidity and mortality, especially in rabbits under four months of age.
The transmission of hepatic coccidiosis occurs through the fecal-oral route when rabbits ingest sporulated oocysts from contaminated environments. Infected rabbits shed oocysts in their feces, which require one to two days in the environment to sporulate and become infectious. Once mature, these oocysts can survive in the environment for extended periods, remaining viable in soil, bedding, and on surfaces for months under favorable conditions. Young rabbits typically become infected soon after weaning when they begin exploring their environment and consuming solid food, though infection can occur at any age. The sporulated oocysts, when ingested, release sporozoites that penetrate the intestinal wall and migrate via the portal bloodstream to the liver, where they invade the bile duct epithelial cells and begin their reproductive cycle.
The impact of hepatic coccidiosis on affected rabbits depends on the parasite burden and the individual rabbit's immune status. Light infections may produce no obvious clinical signs while causing microscopic liver damage detectable only on postmortem examination. Moderate infections cause gradual liver enlargement, reduced growth rates in young rabbits, and intermittent clinical signs. Heavy infections, particularly in young or immunocompromised rabbits, can cause severe hepatic damage with dramatic clinical illness including jaundice, abdominal distension, and potentially fatal liver failure. The parasite's multiplication within bile duct epithelial cells causes progressive damage including bile duct hyperplasia, periductal fibrosis, and characteristic white nodules visible on the liver surface. Secondary complications include disrupted bile flow, compromised liver function, and increased susceptibility to other infections.
Treatment of hepatic coccidiosis is most successful when initiated early, before severe liver damage has occurred. Anticoccidial medications can halt parasite reproduction and allow liver healing in many cases. However, advanced liver damage may be irreversible, and severely affected young rabbits may not survive despite treatment. Prevention through strict hygiene, appropriate stocking density, and environmental management is far more effective than treating established disease. Working with a rabbit-savvy veterinarian is essential for accurate diagnosis, appropriate treatment selection, and development of prevention strategies. Rabbit owners should be aware that hepatic coccidiosis is distinct from intestinal coccidiosis and requires specific diagnostic attention, as the two forms have different clinical presentations and implications.
