Encephalitozoon cuniculi is extremely common in pet rabbits and causes vestibular signs, head tilt, ataxia, and other neurological symptoms that closely mimic brain abscess. Serological testing and response to appropriate treatment help distinguish these conditions. Pasteurella otitis without actual brain involvement can cause vestibular signs similar to those seen with brain abscess. Baylisascaris procyonis infection should be considered in rabbits with outdoor exposure to raccoon habitats. Other causes of central nervous system signs include toxoplasmosis, viral encephalitis, trauma, cerebrovascular accidents, and neoplasia. Advanced imaging and comprehensive diagnostic workup help differentiate these conditions.

Potential complications of brain abscess and its treatment include several serious concerns. GI stasis develops rapidly when affected rabbits stop eating and must be prevented or treated aggressively with syringe feeding and gut motility support. Hepatic lipidosis can develop from prolonged anorexia. Seizures may cause secondary injuries or aspiration. Pressure sores and urine scalding occur in recumbent rabbits without careful nursing care. Long-term antibiotic therapy carries risks including disruption of normal gut flora, potential antibiotic resistance, and rarely drug-specific side effects. Recurrence of infection is possible even after apparently successful treatment, particularly if the primary source was not adequately addressed. Permanent neurological deficits may persist even in rabbits who otherwise recover from the infection.
