Cryptococcosis is a systemic fungal infection caused primarily by Cryptococcus neoformans, an encapsulated yeast organism found worldwide in soil enriched with bird droppings, particularly from pigeons and other avian species. This opportunistic pathogen affects horses through inhalation of fungal spores or yeast cells, establishing infection in the respiratory tract before potentially disseminating to other organs including the central nervous system, skin, and lymph nodes. While relatively uncommon in horses compared to other species, cryptococcosis represents a significant diagnostic and therapeutic challenge when it does occur, often presenting with chronic, progressive symptoms that can be difficult to distinguish from other conditions.
Cryptococcosis occurs sporadically in horses throughout the world, with no particular geographic restriction as the causative organism exists ubiquitously in environments contaminated with avian excreta. The disease has been reported in horses of all ages and breeds, though certain factors including immunocompromise, concurrent illness, and heavy environmental exposure may increase susceptibility. Urban and suburban horses living near roosting sites for pigeons, starlings, or other birds face potentially higher exposure levels, as do horses housed in older barns where bird nesting may occur. The disease appears to be underdiagnosed due to its rarity and the nonspecific nature of early clinical signs.
The impact of cryptococcosis on equine health varies substantially depending on the organ systems involved and the stage of disease at diagnosis. Nasal and upper respiratory infections may cause chronic nasal discharge, facial swelling, and breathing difficulties, while pulmonary involvement produces coughing, exercise intolerance, and respiratory compromise. Most concerning is the organism's predilection for the central nervous system, where cryptococcal meningitis or encephalitis produces neurological deficits ranging from subtle behavioral changes to severe ataxia, seizures, and death. Cutaneous forms cause disfiguring masses and ulcerated lesions that may become secondarily infected.
With appropriate diagnosis and aggressive antifungal treatment, some horses with cryptococcosis achieve clinical resolution, though outcomes depend heavily on disease extent and early intervention. Nasal and cutaneous forms generally carry better prognosis than disseminated disease or central nervous system involvement. Treatment typically requires months of antifungal medication, substantial financial investment, and diligent monitoring. Horse owners should be aware that this condition exists and seek veterinary evaluation for horses showing chronic respiratory symptoms, unexplained neurological signs, or unusual masses, particularly when conventional treatments fail to produce improvement.
