Uterine prolapse is a rare but serious emergency condition in female dogs in which the uterus turns inside out and protrudes through the cervix and vagina to the outside of the body. This condition occurs almost exclusively in the immediate postpartum period, typically within hours to days after whelping. The prolapsed uterus appears as a large, fleshy mass extending from the vulva, and the tissue rapidly becomes swollen, traumatized, and compromised once exposed to the external environment. Without immediate veterinary intervention, uterine prolapse can be fatal.
The mechanism of uterine prolapse involves the continued straining and contractions of labor after puppy delivery has been completed, combined with relaxation of the structures that normally hold the uterus in place within the abdomen. As the dog continues to strain, the uterus can turn inside out starting from the tip of one uterine horn and progressively pulling more tissue out through the dilated cervix. The entire process may happen rapidly, resulting in a startling presentation of a large mass of tissue suddenly appearing at the vulva.
The impact of uterine prolapse on the affected dog is severe and immediate. The exposed uterine tissue quickly becomes edematous, congested with blood that cannot return to circulation, and traumatized by contact with bedding, the ground, and the dog's own licking or chewing. Infection risk is extremely high due to environmental contamination of the normally sterile internal tissue. Hemorrhage may occur from torn blood vessels, and shock can develop from blood loss, toxin absorption, and cardiovascular compromise. The condition is extremely painful and distressing for the affected dog.
Uterine prolapse is a treatable condition when recognized and addressed immediately through emergency veterinary care. Treatment involves either reducing the prolapse by returning the uterus to its normal position or performing an ovariohysterectomy if the tissue is too damaged for replacement. Prognosis depends heavily on how quickly treatment is initiated, the degree of tissue damage at presentation, and the presence of complications such as shock or infection. Rapid response dramatically improves survival rates and outcomes.
