Urethral tumors are malignant neoplasms arising within or extending into the urethra, the tube that carries urine from the bladder out of the body in dogs. While primary urethral tumors are relatively uncommon, the urethra is frequently affected by transitional cell carcinoma originating in the bladder and extending into the urethra, making urethral involvement a significant clinical concern in canine urinary tract oncology. These tumors predominantly affect middle-aged to older dogs, with female dogs more commonly diagnosed than males. The anatomical location of urethral tumors creates particular management challenges, as the urethra cannot be removed without significant impact on urinary function.
The development of urethral tumors involves malignant transformation of the transitional epithelium lining the urethra, the same cell type that lines the bladder. Most urethral tumors represent either primary transitional cell carcinoma arising in the urethra or extension from bladder TCC that has grown into the urethral lumen. Less commonly, squamous cell carcinoma or other tumor types may affect the urethra. The tumor grows within the urethral wall, progressively narrowing the urethral lumen and obstructing urine flow. Without treatment, complete urethral obstruction can develop, creating a life-threatening emergency as urine backs up into the kidneys.
The impact of urethral tumors on affected dogs relates primarily to progressive urinary obstruction and its consequences. Dogs experience increasing difficulty urinating as the tumor narrows the urethral channel. Frequent attempts to urinate with production of only small amounts of urine, straining, and blood in the urine are common presenting complaints. Complete obstruction prevents urination entirely and causes rapid kidney damage if not relieved. Quality of life may be significantly impacted by the constant urge to urinate and associated discomfort. Metastatic spread to regional lymph nodes and distant organs occurs in many cases, adding systemic disease effects.
Treatment of urethral tumors focuses on maintaining urinary patency and controlling tumor progression through various approaches. Complete surgical removal is rarely possible given the anatomical constraints of the urethra. Urethral stenting has emerged as an important palliative option, placing a mesh tube within the urethra to hold it open despite tumor presence. Medical management with NSAIDs and chemotherapy can slow tumor progression. Radiation therapy may provide local tumor control. While cure is rarely achievable, many dogs can maintain acceptable quality of life for months with appropriate multimodal management.
