Thrombocytosis is a hematological condition characterized by an abnormally elevated platelet count in the circulating blood. Platelets, also known as thrombocytes, are small cell fragments produced by megakaryocytes in the bone marrow that play an essential role in hemostasis and blood clotting. In dogs, normal platelet counts generally range from 175,000 to 500,000 per microliter of blood, although reference ranges may vary slightly between laboratories. Thrombocytosis is typically defined as a platelet count exceeding the upper limit of the reference range.
The condition is broadly categorized into two forms: reactive (secondary) thrombocytosis and primary (autonomous) thrombocytosis. Reactive thrombocytosis is by far the more common presentation and occurs as a physiological or pathological response to an underlying condition such as inflammation, infection, iron deficiency, neoplasia, or recent surgery. In these cases, the elevated platelet count is driven by increased production of thrombopoietin and other cytokines that stimulate megakaryocyte proliferation and platelet release.
Primary thrombocytosis, also referred to as essential thrombocythemia, is a rare myeloproliferative disorder in which the bone marrow autonomously produces excessive numbers of platelets without an identifiable external stimulus. This form results from clonal expansion of abnormal megakaryocytes and can lead to extremely high platelet counts that significantly increase the risk of both thrombotic and hemorrhagic complications.
Thrombocytosis is frequently an incidental finding on routine blood work rather than a condition that presents with specific clinical signs. Its clinical significance depends entirely on the underlying cause and the degree of platelet elevation. Mild reactive thrombocytosis in the context of a known inflammatory condition may require no specific treatment beyond addressing the primary disease, whereas primary thrombocytosis or extreme platelet elevations demand thorough investigation and targeted therapeutic intervention.
