Subinvolution of placental sites, commonly abbreviated as SIPS, is a postpartum condition in female dogs characterized by prolonged uterine bleeding following whelping. Under normal circumstances, the areas where placentas attached to the uterine wall undergo a healing process called involution, during which the blood vessels at these attachment sites close and the tissue repairs itself. In dogs with SIPS, this normal healing process fails to occur properly, resulting in persistent bleeding from the placental attachment sites that can continue for weeks or even months after giving birth.
The underlying cause of subinvolution of placental sites involves failure of the normal blood vessel closure that should occur at placental attachment sites after delivery. In healthy postpartum dogs, specialized cells called trophoblasts, which originated from the fetal membranes, are cleared from the uterine wall as part of the healing process. In SIPS, these trophoblast cells persist and continue to invade the uterine blood vessels, preventing the normal clotting and vessel closure that stops postpartum bleeding. The retained cells maintain open channels between the maternal blood supply and the uterine cavity.
The impact of SIPS on a dog's health ranges from minimal concern to potentially significant depending on the severity and duration of bleeding. Many affected dogs experience only mild, persistent spotting that causes no systemic problems and eventually resolves on its own. However, some dogs develop more significant hemorrhage that can lead to anemia, weakness, and potential secondary infections of the reproductive tract. The constant discharge can also create hygiene challenges and may cause skin irritation around the vulva if not properly managed.
Subinvolution of placental sites is generally a treatable condition with good outcomes when properly managed. Mild cases may require only monitoring and supportive care, while more severe cases benefit from medical intervention or surgical treatment. Early recognition of abnormally prolonged postpartum bleeding and veterinary evaluation are important to confirm the diagnosis and rule out other causes of postpartum hemorrhage. Most dogs recover fully and can have normal future pregnancies if spaying is not elected, though some veterinarians recommend spaying affected dogs to prevent recurrence.
