Splenic rupture is a life-threatening emergency that occurs when the spleen tears or breaks open, causing rapid internal bleeding into the abdominal cavity. This condition, also known as hemoabdomen when referring to the blood accumulation in the abdomen, can result from trauma or from spontaneous rupture of splenic masses or tumors. The spleen is a highly vascular organ that stores blood and filters blood cells, making rupture particularly dangerous because massive hemorrhage can occur within minutes. Dogs with splenic rupture require immediate emergency veterinary care and usually need surgical intervention to survive.
The causes of splenic rupture fall into two main categories: traumatic and spontaneous. Traumatic rupture occurs when the spleen is damaged by external forces such as being hit by a car, kicked, or experiencing blunt abdominal trauma. Spontaneous rupture typically results from splenic masses or tumors that weaken the splenic tissue and eventually break through the capsule. Hemangiosarcoma, a malignant cancer of blood vessel walls, is the most common cause of spontaneous splenic rupture in dogs and represents approximately two-thirds of all splenic masses. Benign masses including hematomas and nodular hyperplasia can also rupture but generally carry better long-term prognoses once surgically addressed.
The impact of splenic rupture is immediate and severe, as blood rapidly accumulates in the abdominal cavity. Dogs can lose significant blood volume in a short time, leading to hypovolemic shock, collapse, and death if not treated promptly. The severity depends on the rate and extent of bleeding; some dogs experience gradual oozing that allows partial compensation, while others have catastrophic hemorrhage requiring immediate surgical intervention. Even dogs that initially stabilize may experience re-bleeding episodes until the damaged spleen is removed. The condition is particularly devastating because it often occurs suddenly in dogs that appeared healthy just hours before.
Treatment for splenic rupture centers on emergency stabilization followed by surgical removal of the spleen. Most dogs can live normal lives without a spleen, as other organs can compensate for its functions. However, the prognosis depends heavily on the underlying cause of the rupture. Dogs with traumatic rupture or benign masses often have excellent outcomes after surgery. Those with hemangiosarcoma face a more guarded prognosis because the cancer has often already spread by the time symptoms appear, though surgery combined with chemotherapy can extend quality survival time.
