Nasal aspergillosis is a fungal infection of the nasal cavity and paranasal sinuses caused predominantly by Aspergillus fumigatus, a saprophytic mold found ubiquitously in the environment. The condition represents the most common form of aspergillosis in dogs and is distinct from the less frequently encountered disseminated or systemic form of the disease. Sinonasal aspergillosis is characterized by progressive destruction of the nasal turbinates and mucosal surfaces, leading to chronic nasal discharge, pain, and significant morbidity if left untreated.
Aspergillus fumigatus is a filamentous fungus that produces vast quantities of small, airborne conidia (spores) that are routinely inhaled by dogs and humans alike during normal breathing. In the vast majority of healthy individuals, the innate immune defenses of the nasal mucosa, including mucociliary clearance, secretory immunoglobulins, and resident immune cells, effectively eliminate inhaled spores before infection can establish. Nasal aspergillosis develops when these local defense mechanisms are compromised or overwhelmed, allowing the fungus to colonize the nasal epithelium and initiate tissue invasion.
The infection typically begins in the nasal cavity and may extend to involve the frontal sinuses, creating a chronic, progressive disease process. As the fungus proliferates, it forms visible fungal plaques or mats on the mucosal surfaces and within the turbinate structures. These plaques consist of dense networks of fungal hyphae intertwined with inflammatory debris and necrotic tissue. The enzymatic activity of the growing fungus, combined with the host's inflammatory response, leads to progressive destruction and remodeling of the delicate nasal turbinate architecture.
Nasal aspergillosis should be distinguished from disseminated aspergillosis, a separate and more serious condition in which the fungus spreads beyond the nasal cavity to involve other organ systems including bone, kidneys, spleen, and the vertebral column. Disseminated aspergillosis is most commonly caused by Aspergillus terreus or Aspergillus deflectus and tends to occur in immunosuppressed individuals or breeds with suspected immune dysfunction, particularly German Shepherds. The two forms of aspergillosis differ substantially in their clinical presentation, treatment approaches, and prognosis.
