Mesenteric volvulus is a catastrophic surgical emergency in dogs involving the twisting or rotation of the small intestine around its mesenteric root, which is the attachment point where the intestines connect to the body wall and receive their blood supply. This rotation strangulates the blood vessels supplying the intestines, rapidly cutting off blood flow to large portions of the small intestine. Without immediate blood supply, the intestinal tissue begins dying within hours, releasing toxins into the bloodstream and leading to shock, sepsis, and death if not treated with emergency surgery. Mesenteric volvulus is relatively rare but almost universally fatal without prompt surgical intervention.
The mesentery is a fan-shaped fold of tissue that suspends the intestines within the abdominal cavity and contains the blood vessels, lymphatic vessels, and nerves that supply the intestinal tract. When the intestine rotates around its mesenteric root, these vital structures become compressed and occluded. Arterial blood cannot reach the intestinal tissue, and venous blood cannot drain away, causing rapid tissue death and massive inflammatory response. The twisted loop of intestine also becomes obstructed, preventing normal passage of intestinal contents and causing severe distension. The combined effects of ischemia, obstruction, and systemic toxicity create a life-threatening crisis.
The impact of mesenteric volvulus on affected dogs is profound and rapid. Dogs progress from apparently normal to critically ill within hours. Severe abdominal pain, shock, and collapse occur as the condition advances. The dying intestinal tissue releases bacteria and toxins that enter the bloodstream, causing sepsis and systemic inflammatory response syndrome. Multi-organ failure can develop rapidly without treatment. Even with emergency surgery, mortality rates are significant due to the severity of systemic effects and the necessity of removing large portions of damaged intestine.
Treatment requires immediate surgical intervention to untwist the intestine and restore blood flow while removing any non-viable tissue. Time is the critical factor, as earlier intervention preserves more intestinal tissue and reduces systemic toxicity. Dogs must be aggressively stabilized with intravenous fluids and supportive care before and during surgery. The extent of intestinal damage and the dog's overall condition at the time of surgery significantly influence survival. While mesenteric volvulus is a devastating condition, dogs that receive prompt surgical treatment and survive the immediate post-operative period can recover, though the emergency nature and high stakes make prevention awareness and early recognition essential.
