Melanocytomas are benign melanocytic tumors that may appear identical to melanomas on visual examination but carry an excellent prognosis. Oral epulis and gingival hyperplasia are benign oral growths that can resemble melanoma. Subungual squamous cell carcinoma affects the nail bed similarly to digital melanoma and requires histopathology for differentiation. Other skin tumors including mast cell tumors and soft tissue sarcomas may have similar appearances to cutaneous melanoma. Accurate diagnosis through biopsy and histopathology is essential because treatment approaches and prognoses differ substantially among these conditions.

Potential complications of malignant melanoma include consequences of both the primary tumor and its metastatic spread. Local tumor growth can cause difficulty eating for oral melanomas, lameness and pain for digital tumors, and cosmetic changes for cutaneous masses. Secondary bacterial infection of ulcerated tumors is common and may require antibiotic treatment. Metastasis to regional lymph nodes may cause visible or palpable swelling. Pulmonary metastasis leads to respiratory symptoms that progressively worsen as metastatic burden increases. Rare metastatic sites including brain can cause neurological symptoms. These complications should prompt reassessment of prognosis and treatment goals, with quality of life remaining the paramount consideration in management decisions.
