Lung lobe torsion is a serious and potentially life-threatening condition in which one of the lung lobes twists on its axis, rotating around the bronchus and blood vessels that supply it. This rotation obstructs blood flow and air exchange in the affected lobe, causing rapid congestion, tissue death, and accumulation of fluid in and around the lung. While relatively uncommon compared to other respiratory conditions, lung lobe torsion constitutes a surgical emergency requiring prompt diagnosis and intervention. Without treatment, the condition is invariably fatal, but with timely surgery, most affected dogs can make full recoveries.
The pathophysiology of lung lobe torsion involves mechanical obstruction of both the airway and vascular supply to the affected lobe. When the lobe rotates, the bronchus kinks and closes, preventing air from entering or leaving the lobe. Simultaneously, the pulmonary veins become occluded while arterial blood flow initially continues, causing the lobe to become engorged with blood that cannot drain. The lobe swells dramatically, becoming consolidated and heavy with trapped blood and fluid. Progressive tissue hypoxia leads to cellular death, and inflammatory mediators released from the damaged tissue can cause systemic effects. Pleural effusion, fluid accumulation in the chest cavity around the lungs, commonly develops.
Certain dogs face elevated risk for lung lobe torsion based on body conformation and breed. Large and giant deep-chested breeds, particularly Afghan Hounds, have the highest reported incidence. Interestingly, Pugs, despite their small size and very different body type, also show notably increased susceptibility, possibly related to their unique respiratory anatomy. Other large breed dogs including Great Danes, Irish Wolfhounds, and various sighthounds have been affected. While the condition can occur spontaneously, it may also develop secondary to other thoracic conditions including pleural effusion, pneumonia, trauma, or thoracic surgery, suggesting that increased mobility of lung lobes within the chest cavity may contribute to risk.
The prognosis for lung lobe torsion depends heavily on the speed of diagnosis and surgical intervention. Dogs that receive prompt surgical treatment, typically involving removal of the affected lobe, have good survival rates with most studies reporting success rates of seventy to ninety percent or higher. Delayed diagnosis allows progressive deterioration and increases surgical risk. Dogs that survive surgery typically recover well and can live normal lives with the remaining lung lobes compensating fully for the removed tissue. Understanding this condition helps owners recognize the urgency of respiratory emergencies and the importance of immediate veterinary evaluation.
