Insulinoma is a tumor of the pancreatic beta cells that produces excessive amounts of insulin, leading to dangerously low blood sugar levels. The beta cells in the pancreas normally produce insulin in carefully regulated amounts to maintain blood glucose within a healthy range. When these cells become cancerous and form an insulinoma, they lose normal regulation and continuously secrete insulin regardless of blood glucose levels. This inappropriate insulin secretion causes persistent or episodic hypoglycemia, which can produce a range of symptoms from mild weakness to life-threatening seizures and collapse.
The pancreas contains specialized cell clusters called islets of Langerhans, which include the insulin-producing beta cells. Insulinomas arise from these beta cells and are the most common pancreatic endocrine tumor in dogs. While relatively rare compared to many other canine cancers, insulinomas represent an important diagnosis because of their profound metabolic effects. These tumors are typically small and may be difficult to locate during imaging or surgery. Unfortunately, most canine insulinomas are malignant and have often spread to nearby lymph nodes or the liver by the time of diagnosis.
The impact of insulinoma on affected dogs can be severe and debilitating. The brain relies almost exclusively on glucose for energy, making it particularly vulnerable to low blood sugar. Dogs with insulinoma may experience episodes of weakness, disorientation, trembling, and collapse when blood glucose drops. Seizures occur in a significant portion of affected dogs as the brain's glucose supply becomes critically low. Between episodes, dogs may appear relatively normal, which can delay recognition of the underlying problem. The episodic nature of symptoms often leads to initial misdiagnosis as epilepsy or other neurological conditions.
Despite its serious nature, insulinoma can often be managed effectively, at least for a period of time, through surgery and medical therapy. Surgical removal of the tumor, when possible, provides the best chance for longer-term control. Medical management using dietary modification and medications that counteract insulin's effects helps maintain stable blood sugar when surgery is not possible or as an adjunct to surgical treatment. Early diagnosis and intervention improve outcomes, making awareness of this condition important for owners of middle-aged and older dogs, particularly in breeds commonly affected.
