Hypoadrenocorticism, commonly known as Addison's disease, is an endocrine disorder resulting from insufficient production of hormones by the adrenal glands. These small glands, located near the kidneys, produce cortisol and aldosterone, hormones essential for regulating metabolism, stress response, blood pressure, and electrolyte balance. When the adrenal glands fail to produce adequate amounts of these hormones, dogs develop a constellation of symptoms that can range from vague and intermittent to acutely life-threatening. Often called the great pretender due to its variable and nonspecific presentation, Addison's disease can be challenging to diagnose but responds remarkably well to treatment once identified.
The adrenal glands consist of two distinct regions that produce different hormones. The outer layer, called the cortex, produces glucocorticoids like cortisol, which help the body respond to stress and regulate metabolism, and mineralocorticoids like aldosterone, which control sodium and potassium balance. In typical hypoadrenocorticism, both types of hormones are deficient due to destruction or dysfunction of the adrenal cortex. This dual deficiency leads to metabolic disturbances, electrolyte imbalances, and an inability to mount appropriate stress responses. A less common form, called atypical Addison's disease, involves only glucocorticoid deficiency while mineralocorticoid production remains intact.
The impact of hypoadrenocorticism on a dog's health varies considerably depending on the severity of hormone deficiency and how suddenly the condition develops. Some dogs show chronic, waxing and waning symptoms over months before diagnosis, while others present in acute crisis, called an Addisonian crisis, which constitutes a medical emergency. The electrolyte imbalances, particularly elevated potassium and decreased sodium, can cause life-threatening cardiac arrhythmias and cardiovascular collapse. Without treatment, severe cases can be fatal within hours to days. However, the severity of potential complications contrasts sharply with the excellent outcomes possible once the condition is properly diagnosed and treated.
With appropriate hormone replacement therapy, dogs with Addison's disease can live normal, healthy lives with life expectancy similar to unaffected dogs. Treatment involves replacing the deficient hormones through daily or monthly medications, allowing the body to function normally. Most dogs respond dramatically to treatment, with marked improvement often seen within days of starting therapy. The key to successful management is accurate diagnosis, appropriate initial stabilization if the dog is in crisis, and consistent long-term hormone supplementation. Regular monitoring ensures that medication doses remain appropriate and allows early detection of any complications.
