Hyphema is a serious ocular condition characterized by the presence of blood within the anterior chamber of the eye, which is the fluid-filled space between the cornea at the front of the eye and the iris and lens behind it. This hemorrhage appears as blood pooling in the front portion of the eye and can range from a small streak of blood to complete filling of the anterior chamber, obscuring the iris and pupil from view. Hyphema represents a significant veterinary concern because it indicates bleeding within the eye and can lead to vision impairment, increased intraocular pressure, and permanent ocular damage if not treated appropriately.
The anterior chamber normally contains a clear fluid called aqueous humor, which provides nutrients to structures within the eye and maintains proper intraocular pressure. When blood vessels within the eye rupture due to trauma, inflammation, or other causes, blood enters this space and becomes visible through the cornea. The appearance can range from a diffuse red haze to a distinct layer of blood that settles to the bottom of the anterior chamber when the dog is upright, a phenomenon known as a blood-fluid level.
Hyphema can occur as an isolated finding or may accompany other serious injuries or conditions affecting the eye or the body as a whole. Trauma is the most common cause in otherwise healthy dogs, but hyphema can also signal underlying systemic diseases including clotting disorders, hypertension, or cancer. For this reason, dogs presenting with hyphema require thorough evaluation to determine the underlying cause and to assess for concurrent problems that may affect treatment and prognosis.
Prompt veterinary attention is essential for any dog with hyphema to preserve vision and prevent complications. The blood within the anterior chamber can obstruct vision, damage the cornea, clog the drainage angle leading to glaucoma, and cause inflammation that further compromises ocular health. With appropriate treatment and management of underlying causes, many dogs with hyphema recover good vision, though the prognosis depends significantly on the cause and severity of the hemorrhage as well as how quickly treatment is initiated.
