Granulomatous hepatitis is an inflammatory liver condition in dogs characterized by the formation of granulomas within the hepatic tissue. Granulomas are organized collections of immune cells, primarily macrophages that have transformed into epithelioid cells, often surrounded by lymphocytes and other inflammatory cells. These structures form as the immune system attempts to wall off and contain substances it cannot easily eliminate, including certain infectious organisms, foreign materials, or in some cases, the body's own tissues perceived as threats. Granulomatous hepatitis can occur as an isolated liver condition or as part of systemic granulomatous disease affecting multiple organs throughout the body.
The mechanism underlying granuloma formation involves a specific type of immune response known as cell-mediated or type IV hypersensitivity. When macrophages encounter certain pathogens or antigens they cannot destroy through normal phagocytosis, they transform into specialized epithelioid cells and fuse to form multinucleated giant cells. Additional immune cells including T lymphocytes surround these cells, and fibroblasts may lay down collagen creating a fibrous capsule around the granuloma. This process represents the immune system's attempt to contain what it cannot eliminate. In the liver, granulomas can be distributed throughout the hepatic parenchyma, concentrated in portal areas, or found in specific patterns depending on the underlying cause. The granulomatous inflammation displaces normal liver tissue and can impair hepatic function depending on the extent of involvement.
The impact of granulomatous hepatitis on affected dogs ranges from subclinical disease detected incidentally to severe hepatic dysfunction with systemic illness. When granulomas are small and few in number, they may cause minimal disruption to liver function and produce no clinical signs. More extensive granulomatous inflammation interferes with normal hepatocyte function, bile flow, and hepatic circulation, leading to progressive liver disease. Dogs with systemic granulomatous conditions often have multiorgan involvement, resulting in varied clinical presentations depending on which organs are most affected. The underlying cause significantly influences both the clinical course and the impact on the individual patient. Infectious causes may produce acute, severe illness, while idiopathic granulomatous disease may progress more slowly over months to years.
Treatment of granulomatous hepatitis depends entirely on identifying and addressing the underlying cause. Infectious granulomatous hepatitis requires specific antimicrobial therapy targeting the causative organism, whether bacterial, fungal, parasitic, or protozoal. Idiopathic or immune-mediated granulomatous disease typically requires immunosuppressive therapy to control the aberrant immune response forming granulomas. The prognosis varies considerably based on the cause, extent of liver involvement, and response to treatment. Early and accurate diagnosis is essential for successful management. Veterinary expertise in infectious disease and hepatology is often necessary for complex cases. With appropriate treatment, many dogs with granulomatous hepatitis can achieve remission or cure, though some require long-term therapy.
