Eosinophilic myositis is an inflammatory condition in which eosinophils, a specialized type of white blood cell, infiltrate skeletal muscle tissue and cause progressive damage. In dogs, this disease most commonly targets the muscles of mastication, which are the muscles responsible for chewing and jaw movement. The temporalis, masseter, and pterygoid muscles are frequently affected, though the condition can occasionally extend to muscles throughout the body, including the limbs and trunk.
The underlying mechanism of eosinophilic myositis involves an aberrant immune response in which the body's defense system mistakenly identifies proteins within the muscle fibers as foreign. The masticatory muscles contain a unique type of myofiber known as type 2M fibers, which express proteins not found in other skeletal muscles. This distinct protein composition makes these muscles particularly vulnerable to immune-mediated attack, and the resulting inflammatory cascade draws large numbers of eosinophils into the muscle tissue.
Eosinophilic myositis can present in two clinical phases. The acute phase is characterized by painful swelling of the affected muscles, difficulty opening the mouth, and systemic signs of illness. If the condition goes untreated or recurs through multiple flare-ups, it can progress to a chronic phase in which the inflamed muscle tissue is replaced by fibrous scar tissue. This fibrosis leads to irreversible muscle atrophy and permanent restriction of jaw movement.
While eosinophilic myositis can affect dogs of any age or breed, it is most commonly diagnosed in young to middle-aged large-breed dogs. The condition is distinct from other forms of myositis, such as polymyositis or infectious myositis, and requires specific diagnostic testing and targeted immunosuppressive therapy to achieve effective management.
