Dermoid sinus, commonly abbreviated as DS, is a congenital defect in which a tubular indentation of the skin extends inward from the dorsal midline toward the spinal column. The sinus is a remnant of an incomplete separation between the skin (ectoderm) and the neural tube during embryonic development. This tube-like structure is lined with skin tissue, including hair follicles and sebaceous glands, and can extend to varying depths ranging from the superficial subcutaneous tissue to the dura mater or even the spinal cord itself.
The condition is most closely associated with the Rhodesian Ridgeback breed, where it has been recognized and studied for decades. The relationship between the ridge gene and dermoid sinus is a subject of ongoing genetic research. The dorsal ridge characteristic of the breed results from a duplication of a region on canine chromosome 18, and the same genetic region has been implicated in the development of dermoid sinus, suggesting that the ridge and the predisposition to DS are genetically linked.
Dermoid sinus can occur anywhere along the dorsal midline from the base of the skull to the tail, though certain locations are more common. The cervical and cranial thoracic regions are the most frequently affected sites. The condition may be single or multiple, and bilateral occurrence is possible though less common. Each sinus operates as an independent structure and may vary in depth, size, and clinical significance even when multiple sinuses are present in the same animal.
While dermoid sinus is often described as a condition specific to ridged breeds, it has been documented in other breeds and mixed-breed dogs as well, albeit at much lower frequency. In non-ridged breeds, the condition appears to arise from different developmental mechanisms, though the basic pathology of incomplete ectodermal-neural tube separation is the same. Regardless of breed, dermoid sinus requires veterinary evaluation and, in most cases, surgical intervention to prevent potentially life-threatening complications.
