What Is a Dermoid Sinus?

A dermoid sinus is a congenital defect consisting of a tubular tract lined with skin-like tissue that extends inward from the surface of the skin along the dorsal midline toward deeper structures, potentially reaching as far as the spinal cord and its surrounding meninges. Unlike a closed dermoid cyst, a dermoid sinus maintains an open or partially open connection to the skin surface, often visible as a small opening, dimple, or tuft of hair protruding from the midline of the back, neck, or head.

The condition results from incomplete separation of the skin ectoderm from the neural tube during embryonic development. Normally, as the neural tube closes and sinks beneath the surface, the overlying ectoderm fuses seamlessly above it to form continuous skin. When this separation is imperfect, a bridge of ectodermal tissue remains connecting the skin surface to the deeper structures along the neural axis. This persistent connection becomes the dermoid sinus tract, which is lined by stratified squamous epithelium and contains hair follicles, sebaceous glands, and accumulated keratinous debris.

Dermoid sinuses are classified into five types based on the depth to which the tract extends. Type I sinuses connect the skin to the supraspinous ligament. Type II sinuses extend to but do not penetrate the supraspinous ligament. Type III sinuses pass through the ligament and attach to the dura mater surrounding the spinal cord. Type IV sinuses penetrate the dura and communicate directly with the subarachnoid space. Type V sinuses are true dermoid cysts that lie along the midline without a surface opening but are embedded within the dura. This classification system has direct implications for surgical planning and prognosis.

The clinical significance of a dermoid sinus depends heavily on its type and whether complications have developed. Superficial sinuses may cause only cosmetic concern or minor local irritation, while deep sinuses that communicate with the spinal canal represent a serious threat to the dog's neurological health due to the risk of ascending infection leading to meningitis or myelitis.

Causes and Embryological Development

The formation of a dermoid sinus is directly linked to errors during neurulation, the embryonic process by which the neural plate folds into the neural tube. During normal development in the early weeks of gestation, the neural plate, a thickened band of ectodermal cells along the embryo's dorsal surface, elevates at its lateral margins to form neural folds. These folds rise, converge, and eventually fuse at the dorsal midline to create the closed neural tube that will become the brain and spinal cord. Simultaneously, the surface ectoderm above the neural tube must separate from the neuroectoderm and close over the top to form an unbroken layer of skin.

When the separation between the surface ectoderm and the closing neural tube is incomplete, a persistent connection of ectodermal tissue remains between the two structures. This residual bridge of tissue becomes the dermoid sinus tract. The cells lining this tract differentiate into mature skin tissue, complete with hair follicles and glands, creating a tube of skin that runs from the body surface deep into the subcutaneous tissue and potentially all the way to the dural covering of the spinal cord.

The genetic basis for dermoid sinus formation has been most extensively studied in the Rhodesian Ridgeback. Research has identified a large genomic duplication on chromosome 18 that is responsible for the breed's characteristic dorsal ridge of hair. This same duplication is strongly associated with dermoid sinus susceptibility. The ridge is inherited as a dominant trait, and homozygous ridged dogs appear to be at higher risk for dermoid sinus than heterozygous individuals, though the exact penetrance and expressivity of the dermoid sinus phenotype vary.

The Thai Ridgeback and Phu Quoc Ridgeback, which share the dorsal ridge phenotype through convergent or shared genetic origins, also exhibit increased dermoid sinus incidence. Outside the ridgeback breeds, sporadic cases occur in Kerry Blue Terriers, Shih Tzus, Yorkshire Terriers, Boerboels, and occasionally other breeds and mixed breed dogs. In non-ridgeback breeds, the genetic mechanisms are less well understood, but the consistent dorsal midline location of the defect points to a common embryological pathway regardless of breed.

Classification and Types

The classification of dermoid sinuses into five types, originally described by Mannion and colleagues and refined by subsequent investigators, provides a standardized framework for describing the depth and anatomical relationships of the sinus tract. This classification is essential for surgical planning, risk assessment, and prognostic communication with owners. Each type represents a different degree of developmental failure and carries distinct clinical implications.

Type I dermoid sinuses are the most superficial variety. The sinus tract extends from the skin surface through the subcutaneous tissue and attaches to the supraspinous ligament, the thick fibrous band that connects the dorsal spinous processes of the vertebrae. Type I sinuses do not penetrate the ligament or enter the vertebral canal. They are the most common type encountered clinically and generally carry the best prognosis, as complete surgical excision can be achieved without the need to access the spinal canal.

Type II sinuses are similar to Type I in that they extend to the supraspinous ligament, but they do not form a direct attachment. Instead, the tract terminates in the subcutaneous tissue near the ligament, connected by a fibrous band rather than a continuous epithelial tube. Type II sinuses are somewhat less common than Type I and also carry a favorable prognosis, though care must be taken during surgery to follow and remove the entire fibrous connection to prevent recurrence.

Type III and Type IV sinuses represent progressively more serious forms of the condition. Type III sinuses penetrate through the supraspinous ligament and attach to the dura mater, the tough outer membrane surrounding the spinal cord. Type IV sinuses go further, penetrating through the dura to communicate directly with the subarachnoid space that contains cerebrospinal fluid. Both types carry a significant risk of ascending infection from the skin surface to the meninges and spinal cord. Type V dermoid sinuses are unique in that they lack a surface opening and exist as isolated, encapsulated cystic structures embedded within the dura, essentially representing intradural dermoid cysts that are discovered incidentally or when they cause spinal cord compression.

Signs and Symptoms

The most characteristic clinical sign of a dermoid sinus is a visible opening, dimple, or tuft of abnormally growing hair on the dorsal midline of the dog, typically located on the neck, back, or head. Palpation of the area often reveals a cord-like structure extending from the skin surface into the deeper tissue, which can sometimes be lifted away from the underlying structures in superficial types. The opening may discharge small amounts of waxy, sebaceous material, and hair may protrude from or surround the sinus opening.

Many dogs with uncomplicated dermoid sinuses show no systemic signs of illness. The sinus may be detected by the breeder during neonatal examination, by the veterinarian during routine wellness visits, or by the owner during grooming. In young puppies, the sinus opening may be very small and difficult to detect without deliberate, careful palpation of the dorsal midline. As the dog grows, the sinus may become more prominent and easier to identify.

When a dermoid sinus becomes infected, the clinical picture changes dramatically. Bacterial colonization of the sinus tract leads to local swelling, pain, warmth, and erythema around the sinus opening. Purulent discharge, which may contain hair and keratinous debris, may drain from the sinus. The dog may exhibit signs of discomfort such as yelping when touched along the back, reluctance to flex the neck or spine, and changes in posture or gait. Recurrent episodes of infection and drainage are common if the sinus is not surgically removed.

The most serious symptoms arise when infection ascends through a deep sinus tract to reach the meninges or spinal cord. Meningitis presents with fever, severe cervical or spinal pain, hyperesthesia where the dog reacts intensely to light touch, neck rigidity, and depression. Myelitis, or inflammation of the spinal cord itself, can produce neurological deficits including ataxia, weakness or paralysis of the limbs, loss of proprioception, and in severe cases, recumbency and loss of bladder or bowel control. These neurological complications represent a medical emergency and can be rapidly fatal without aggressive treatment.

Diagnosis

Diagnosis of a dermoid sinus begins with a systematic physical examination with particular attention to the dorsal midline. The veterinarian inspects the skin from the occiput to the sacrum, looking for dimples, pores, tufts of abnormal hair, or areas of discharge. Palpation is performed by gently rolling the skin between the fingers along the midline, feeling for cord-like structures that extend from the skin surface into deeper tissue. In puppies, the sinus may be very subtle, and examination under good lighting with the puppy in a relaxed state improves detection sensitivity.

Once a suspicious structure is identified, further characterization is necessary to determine the type and extent of the sinus. Injection of sterile contrast material into the sinus opening, followed by radiography (sinography or fistulography), can delineate the course and depth of the tract. This technique involves carefully cannulating the sinus opening with a small catheter, injecting iodinated contrast medium, and taking lateral and ventrodorsal radiographs to visualize the tract's path and its relationship to the vertebral column and spinal canal.

MRI can distinguish between the different sinus types, identify secondary changes such as meningeal enhancement suggesting infection, and detect intradural dermoid cysts that have no surface connection. CT scanning provides complementary information, particularly regarding bony anatomy and the relationship of the tract to vertebral structures.

In neonatal screening programs, diagnosis relies primarily on careful physical examination, as advanced imaging is impractical for routine screening of entire litters. Experienced breeders and veterinarians can detect the majority of dermoid sinuses through palpation alone, though very small or deep sinuses may be missed on initial examination. Puppies from predisposed breeds should ideally be examined multiple times during the first weeks of life, as some sinuses become more apparent as the puppy grows and the dorsal tissues develop.

Surgical Treatment

Surgical excision is the definitive treatment for dermoid sinuses and should be performed as early as possible once the diagnosis is established, before infection or other complications develop. The goal of surgery is the complete removal of the entire sinus tract from the skin surface to its deepest point of attachment, including all epithelial lining, associated adnexal structures, and any fibrous connections to deeper tissues. Incomplete excision invariably leads to recurrence.

For Type I and Type II sinuses, the surgical procedure involves an elliptical skin incision around the sinus opening, followed by careful dissection of the tract through the subcutaneous tissue down to its attachment at or near the supraspinous ligament. The tract is dissected free using a combination of sharp and blunt technique, maintaining traction on the tract to keep it under tension and facilitate identification of the tissue plane. The portion of supraspinous ligament to which the tract is attached is excised en bloc with the sinus, and the defect is closed in layers.

Type III and Type IV sinuses require a more extensive surgical approach because the tract extends through the supraspinous ligament to reach the dura mater or subarachnoid space. Surgery for these types typically involves a dorsal laminectomy to gain access to the vertebral canal and permit visualization of the dural attachment. The tract is carefully dissected from the dura, and if the dura has been penetrated, the defect must be repaired to prevent cerebrospinal fluid leakage. These procedures are technically demanding and carry risks including hemorrhage, cerebrospinal fluid fistula, neurological injury, and postoperative infection.

Postoperative management following dermoid sinus excision includes appropriate analgesia, antibiotic therapy particularly for infected sinuses or deep excisions, strict activity restriction during the healing period, and wound protection with an Elizabethan collar. Dogs undergoing laminectomy for deep sinus types require more intensive postoperative monitoring, potentially including neurological assessments, and a longer period of confinement and restricted activity. Follow-up examinations should assess wound healing, monitor for signs of recurrence, and evaluate neurological function in cases involving spinal canal access.

Prognosis and Recovery

The prognosis for dogs with dermoid sinuses is strongly influenced by the type of sinus, the presence or absence of complications at the time of surgery, and the completeness of surgical excision. Type I and Type II sinuses that are completely excised before the development of infection carry an excellent prognosis, with low recurrence rates and rapid return to normal function. Most dogs with superficial sinuses recover uneventfully from surgery and experience no long-term consequences from the condition.

Type III sinuses carry a good to guarded prognosis depending on whether infection has occurred. When excised before infection develops, the outcome is generally favorable, though the more complex surgery carries incrementally higher risks of complications compared to superficial sinus excision. If meningitis has already developed at the time of surgery, the prognosis becomes more guarded, as the neurological infection requires aggressive medical management in addition to surgical source control, and permanent neurological deficits may result.

Type IV sinuses represent the most serious category among those with a surface connection. The direct communication between the skin surface and the subarachnoid space creates the highest risk for meningitis and myelitis. Even with successful surgical excision of the sinus, dogs that have experienced meningitis or spinal cord inflammation may retain permanent neurological deficits. The prognosis for Type IV sinuses is guarded, and owners should be counseled about the potential for significant morbidity and the possibility of a prolonged and intensive recovery period.

Recurrence following surgery is most likely when the sinus tract is not completely excised. The reported recurrence rate varies in the veterinary literature but is generally low when surgery is performed by experienced surgeons using appropriate preoperative imaging to define the full extent of the tract. Long-term follow-up should include periodic examination of the surgical site and the dorsal midline for evidence of recurrence or the presence of additional sinuses that may not have been detected initially.

Breed Predispositions

The Rhodesian Ridgeback is overwhelmingly the breed most associated with dermoid sinus. The condition has been recognized in this breed for decades and is directly linked to the genetic variant that produces the characteristic dorsal ridge of reversed hair growth. Research has demonstrated that the ridge is caused by a large duplication on chromosome 18 encompassing several genes, and this duplication is a major risk factor for dermoid sinus development. Prevalence estimates in Rhodesian Ridgeback litters range from five to ten percent in some studies, though this varies by geographic region and breeding population.

The Thai Ridgeback, a breed originating in Thailand that also possesses a dorsal ridge, exhibits a similar predisposition to dermoid sinus. The genetic basis for the ridge in Thai Ridgebacks appears to involve the same or a closely related chromosomal region as in the Rhodesian Ridgeback, resulting in a comparable susceptibility to dermoid sinus formation. The Phu Quoc Ridgeback, a rare breed from Vietnam with a dorsal ridge, is also considered to be at elevated risk, though published data on dermoid sinus prevalence in this breed are limited.

Outside the ridgeback breeds, dermoid sinuses have been reported sporadically in a number of other breeds. Kerry Blue Terriers appear in the veterinary literature with sufficient frequency to suggest a breed predisposition that is independent of the ridge-associated genetic variant. Shih Tzus, Yorkshire Terriers, and Boerboels have also been reported with dermoid sinuses, and occasional cases are documented in a variety of other purebred and mixed breed dogs. In these non-ridgeback breeds, the genetic mechanism is unclear, but the consistent midline location of the defect implies a shared embryological vulnerability.

Breeding recommendations for predisposed breeds emphasize screening all puppies in each litter for dermoid sinuses and removing affected dogs from breeding programs. Some breed organizations have implemented mandatory screening protocols, and responsible breeders have their litters examined by a veterinarian experienced with the condition within the first week of life. While the ridge trait is considered essential to the breed standard of the Rhodesian Ridgeback, ongoing efforts seek to reduce dermoid sinus prevalence through selective breeding without eliminating the ridge itself.

Complications of Untreated Dermoid Sinus

The most devastating complication of an untreated dermoid sinus is ascending infection leading to meningitis, an infection and inflammation of the protective membranes surrounding the brain and spinal cord. Because the dermoid sinus tract provides a direct conduit from the bacteria-laden skin surface to the deeper tissues and potentially to the subarachnoid space, pathogens can travel along the tract to infect the meninges. Meningitis in dogs presents with severe pain, fever, neck rigidity, altered mental status, and can progress to seizures, coma, and death if not treated promptly and aggressively.

Myelitis, or infection and inflammation of the spinal cord itself, represents an even more catastrophic complication that can develop when bacteria penetrate the dura and directly affect spinal cord tissue. Myelitis produces neurological deficits that correspond to the level of spinal cord involvement, including weakness or paralysis of the limbs, loss of proprioception and coordination, urinary and fecal incontinence, and loss of deep pain perception in severe cases. Even with intensive treatment, myelitis can result in permanent neurological disability.

Chronic, recurrent local infection of the sinus tract is a common consequence of leaving a dermoid sinus untreated. The enclosed tract accumulates keratinous debris and sebaceous secretions that serve as a growth medium for bacteria. Repeated episodes of infection, swelling, drainage, and temporary resolution create a cycle of chronic inflammation that progressively worsens the local tissue condition. Dense fibrosis and scarring develop around the sinus tract, making eventual surgical excision more difficult and increasing the risk of complications during the procedure.

Abscess formation in the subcutaneous tissue or deeper along the sinus tract can occur when infection becomes walled off by the body's inflammatory response. These abscesses may drain spontaneously through the sinus opening or through secondary fistulous tracts that develop in the surrounding tissue. Large or deep abscesses may require surgical drainage in addition to eventual sinus excision, and they can compress adjacent structures causing pain and functional impairment.
